Objective:To treat PAR patients with Novo-He1isen Depot(NHD )specific immunotherapy, and clinically observe the curative effect and side reaction of the treatment for those who are allergic to one or more allergens like house dust mite, pollen, fungus, etc, aiming to explore whether NHD could react crossly with the specific antibody of allergen and the possible immunity mechanism so as to provide the clinical allergen-specific immunotherapy for PAR with clinical and theoretical basis.Methods :1.Taking 120 perennial allergic rhinitis ,forty of 120 cases are only sensitive to house dust and randomly divided into group A and group B, other forty are just sensitive to pollen and randomly divided into group C and group D. And the rest are fungusⅠand divided into group E and group F at random. 20 normal students as experiment objects .Group A , group C and group E administrate NHD and Loratadine was adopted to the rest respectively. Comparing and observing symptom, physical sign and curative effect before and after therapy. And the levels of serum IL-4, IL-10,IFN-γand TGF-β1 were assayed by ELISA .2.Blood plasma were collected and peripheral blood molecular cells were separated of pre and post therapy one year of group A,C,E and two weeks of group B,D,F by PBMCS post therapy respectively and cultured respectively in vitro for 24,48,72 hours. Detecting cell proliferation by MTT method, IFN-γ,IL-4,IL-10 and TGF-β1 in supernatant were assayed by ELISA. SIgE and NO were detected by ImmunoCAP and nitric oxide synthetase respectively.Results:After NHD immunotherapy for group A,C,E for one year, the levels of supernatant IFN-γ,TGF-β1 are higher than before. The levels of supernatant IL-4 and IL-10 are obviously lower than before. The serum of SIgE and NO are obviously reduced too. There was a significant difference before and after immunotherapy and controlled one by statistics evaluation (P﹤0.01). Group B,D,F was no significant difference before and after therapy in Group Loratadine by statistics evaluation (P﹥0.05).There was no significant difference between the Group Loratadine after therapy and controlled one by statistics evaluation (P﹥0.05). And there was a significant difference between Group immunotherapy and Group Loratadine by statistics evaluation (P﹤0.01).Conclusions:1. After NHD immunotherapy can modulate PAR patient imbalanced status of Th1/Th2 cells, represent of Th1 As a result of the up-regulation of Th1 or the down-regulation of Th2 or the modulation of both, the serum of SIgE are obviously lower than before. These explain house dust mite ,pollen and fungus contain common epitope or common antigen existing in different allergen. All the allergen mentioned above can combine specific antibody and conduct cross-reaction. That it is protective IgG sub-antibody engender cross-reaction with different allergens. Having an effect of blocking various antigens.2. After NHD immunotherapy of PAR ,the serum of NO was lower than before, Which showing NHD can reduce mastocyte and accordingly cytokine of PAR patients,. Induce nitric oxide synthetase and endothelial nitric oxide synthetase were repressed directly by NHD or by repressing nasal cavity epicyte, nasal sinuses epicyte ,submucous gland epithelial cells , vascular endothelial cell and gland nerve fibers, therefore reducing the synthetase of nitric oxide. To explore and detect change and rule of nitric oxide in PAR patient and then educe normal and abnormal range, which will contribute to monitoring state of PAR and evaluation result of NHD immunotherapy PAR.3. The test is marked with adopting single house dust mite immunotherapy which could alleviate financial burden of PAR patients and has a wide application. However, immunoblotting, Western blot and ELISA inhibition test are still needed to explore mechanism and degree of cross-reacting antigen to confirm the sequence of common epitope and specific target site.
