Clinical Observation Of Shugan Jiangzhuo Decoction In Nonalcoholic Fatty Liver Disease

Objective:To observe the clinical effect and safety of Shugan Jiangzhuo Decoction in treating nonalcoholic fatty liver disease(NAFLD).Methods:The 70 patients selected were all cases of hepatic department of Hubei Hospital of Traditional Chinese Medicine,including outpatients and inpatients.All of them were in accord with Diagnostic Criteria of Nonalcoholic Fatty Liver Disease formulated by the Liver Disease Branch of Chinese Medical Association in 2003.And according to TCM,they belonged to the type of liver depression and spleen deficiency,phlegm resistance.All of them were randomly divided into two groups:treatment group(Shugan Jiangzhuo Decoction group) and control group(UDCA group),35 cases in each group.Both groups were given low salt,low sugar,low fat diet,and taken out of service of other drugs against the disease.The treatment group would be used Traditional Chinese Medicine Shugan Jiangzhuo Decoction for treatment,oral use,twice a day,one decoction per day(200ml),while the control group to ursodeoxycholic acid capsules,oral use,once a pill(250mg),three times a day.The therapeutic effect would be assessed after three months for both groups.Observe the major symptoms and signs,as well as liver function(ALT,AST,γ-GT),blood lipids(TC,TG) and the ratio of liver to spleen CTN(computed tomography number) before and after the treatment.Then evaluated the efficacy according to corresponding medical standards,And observed the blood,urine,feces routine examinations,electrocardiogram,renal function and adverse reactions to evaluate its security.After the observation,all the data would be for statistical analysis,using two-sided test.Mean±standard deviation((?)±s) was used to express measurement data.T test was used for measurement data,whileχ~2 test for numeration data.P＜0.05 means statistically significant difference,P＜0.01 indicating a very significant difference,while P＞0.05 indicates no significant difference.Results:1.The total effective rate of the treatment group is 88.57%,while the control group is 71.43%.The therapeutic effect of two groups have significant difference(P＜0.05),which shows that the treatment group has better total effective rate than the control group.2.The treatment group has a significant difference with the control group in TCM symptom score after treatment(P＜0.05).The treatment group has a very significant difference before and after treatment (P＜0.01),while the control group has a significant difference before and after treatment(P＜0.05).3.The treatment group has a significant and better improvement than the control group in ALT and AST(P＜0.05),while theγ-GT of two groups has no significant difference(P＞0.05).Both of the two groups have significant difference in ALT,AST andγ-GT before and after treatment(P＜0.05).4.The treatment group has a significant and better change than the control group in TC and TG(P＜0.05),and both groups have significant difference in TC and TG before and after treatment(P＜0.05).5.The treatment group has a significant and better increase in the ratio of liver to spleen CTN(computed tomography number) than the control group after treatment(P＜0.05),while the control group has no significant difference before and after treatment(P＞0.05).6.The blood,urine,stool routine examinations,renal function and ECG of patients of two groups are normal before and after treatment.During the treatment,mild diarrhea appeared on one patient of the treatment group,which alleviated after treatment.It may be associated with Cassia obtusifolia L and Trichosanthes kirilowii Maxim,both having the function of loosening lowel to relieve constipation.It indicates that Shugan Jiangzhuo Decoction is safe and effective,having no significant side effects in treating NAFLD.Conclusion:The clinical observation shows that the treatment group used Shugan Jiangzhuo Decoction treating NAFLD has positive and better effect in improving the symptoms,signs,ALT,AST,blood lipids(TC,TG) and the ratio of liver to spleen CTN(computed tomography number) than the control group.Moreover,it is safe and effective,having no significant side effects.It provides us a useful treatment for NAFLD.
