Effect Of Xiaoyu Zhixue Tablet On The Expression Of Platelet Membrane Glycoprotein Ib/IX/V Complex In Patients With Chronic Renal Failure

Objective: To investigate the differences of platelet maximum aggregation rate(MAR), the expressions of platelet membrane glycoprotein (GP) Ib/IX/V complex and GP Ibαbetween patients with chronic renal failure (CRF) and healthy people, and the changes of the above indicators after the treatment of Xiaoyu Zhixue Tablet (消瘀止血片,XYZXT) in order to explore the possible mechanism of hemorrhagic trend in CRF patients and evaluate the therapeutic effect of XYZXT.Methods: Fifty-one patients diagnosed as CRF were enrolled in this study. Flow cytometry was used to assess the expressions of platelet GP Ib/IX/V complex and GP Ibα, and turbidity method was used to determine platelet MAR before and after the treatment of XYZXT. During the course, the indexes of serum creatinine (Scr) and blood urea nitrogen (BUN) were measured. The same indicators were detected in a control group of 31 healthy subjects and compared with those in CRF patients.Results: Compared with control group, the expressions of GP Ib/IX/V complex and GP Ibαand platelet MAR in CRF patients were significantly lower (P = 0.007,P = 0.001 and P = 0.009, respectively) before treatment; after the treatment of XYZXT, the above indicators in CRF patients remarkably increased (P = 0.033,P = 0.026 and P = 0.045, respectively). Though still lower than those in control group, the differences were statistically insignificant. Correlation analysis revealed that the expressions of GP Ib/IX/V complex and GP Ibαwere negatively related to Scr (P = 0.041 and P = 0.026, respectively). There were no statistically significant changes in Scr and BUN before and after treatment.Conclusion: (1) As the receptor of both thrombin and von Willebrand factor, the expression of platelet GP Ib/IX/V complex decreased in CRF patients and resulted in platelet aggregation dysfunction, which might be one of the reasons for hemorrhagic trend in CRF. (2) Platelet GP Ib/IX/V complex and its component GP Ibαdecreased as the level of Scr increased, which might explain why hemorrhage in CRF patients most frequently appeared in uremia. (3) XYZXT was able to upgrade the expressions of GP Ib/IX/V complex and GP Ibα, effectively improve platelet aggregation function and downregulate platelet activation in CRF patients. (4) Our research primarily indicated that XYZXT had no obvious nephrotoxicity.
