Objective:To test levels of leptin in patients with impaired glucose regulation(IGR) and nonalcoholic fatty liver disease(NAFLD) and investigate the correlation between the leptin and insulin resistance.Methods:Clinical and laboratorial datas were collected from thirty healthy controls(NC group),fifty-eight cases of IGR without NAFLD,including twenty-eight cases of impaired fasting glucose(IFG+no FL group),thirty cases of IFG and impaired glucose tolerance(IGT+no FL group).Sixty-three cases of IGR with NAFLD,including thirty-one cases of IFG(IFG+NAFLD group),thirty-two cases of IFG and IGT(IGT+NAFLD group). Fasting plasma glucose,insulin,leptin and lipid profile were measured.Measuring height, weight,waistline,hip circumference,hemeostasis modle assessment(HOMA-IR).We investigated the leptin levels of change and the correlations between these parameters of insulin resistance(Body mass index BMI,waist hip ratio WHR,triglycerides TG,HDL cholesterol HDL-C and LDL cholesterol LDL-C).Result:In patients with IGR,the serum FPG levels,WHR,HOMA-IR were statistically significantly higher.The serum leptin levels were higher in patients with NAFLD than without. A statistically positive correlation was found between leptin and BMI,WHR,TG in NAFLD patients with IGR.The serum leptin levels were negative correlation with HDL-C in NAFLD patients with IGR.The relationship still exited after controling for age,sex,BMI.the serum leptin levels were positive correlation with TG,LDL and HOMA-IR.HOMA-IR and LDL-C play the major role in affecting leptin in NAFLD patients with IGR.Conclusion:In patients with IGR,the serum leptin levels were statistically significantly higher than healthy controls,and the occurrence of NAFLD or deteriorated glucose intolerance only made the situation worse.High leptin,intolerance of glucose-lipid and insulin resistance interact with on another and may eventually induce NAFLD and diabetes.
