Establish The Preliminary PRO Scale Of Key-symptoms In TCM Which Utend For COPD And Pulmonary Heart Disease

Speciality:Respiration of TCMAuthor:Luming ChenTutor:Hui YuResearch objectives:Based on the etiopathogenisis and pathogenesis of chronic obstructive pulmonary disease(COPD) with pulmonary heart disease in traditional Chinese medicine theory,the author built a patient-reported outcome(PRO) scale for the patients,which is used to assess the clinical effectiveness in traditional Chinese medical treatment and to explore an appropriate evaluation for clinical effectiveness of traditional Chinese medicine(TCM).Research methods:After studying the theoretical framework of PRO scales concerning COPD with pulmonary heart disease,retrieving the ancient and modern literature database, and extracting main clinical symptoms of this disease,the project researcher initially got a number of key words relating to the main symptoms.Integrate these key-symptoms,then carry out the preliminary experts'advice investigations and patient self-report questionnaire about the the main symptoms of the disease.And then collect the data and combine the two literature database to determine the main clinical symptoms items preliminarily.At the same time,collecte the everyday words in Mandarin which reflects the frequency as the items to have an investigation which measure the extent of frequency.After statistical analysis,the author will select five correct and equidistant frequency scale level terms,then carry out the second round experts'advice investigations and patient self-report questionnaire.Collect the feedback data to statistics and analyze.At last, the core group determinates the more appropriate key- symptoms as the items in the scale with the collective discussions.Then match these 5 words to the key-symptoms to establish the ultimate PRO scale.Results:15 key-symptoms are confirmed as the main clinical symptoms items of the initial scale:gasp,cough,stethocatharsis,lassitude and debility, chest distress,wheezy phlegm in the throat,easy to to have a cold,oral lips cyanosis,fidgety,puffiness,cardiopalmus,depression,anorexy,can not lie down on the back,hyperhidrosis。5 equidistant words are choosen to represent the frequency degree:never,seldonm,sometimes,often,all the time.Match these words to the key-symptoms and to establish the preliminary PRO scale.Conclusion:with the development of chronic disease clinical estimate system,clinical estimate index also gradually pay more attention to estimating patient's self subjective feeling than the objective biology index of the diease.patient-reported outcome(PRO) scale is widely applicated in clinical estimate system increasingly,which is more comprehensive,convenient,and more relevant to the disease prognosis.So we establish a patient-reported outcome(PRO) scale,which is used to assess the clinical effectiveness in traditional Chinese medical treatment.it colud be an important complementarity for the curative effect evaluation.And the research method also could be generalized as a reference for quantitating other chronic disease.
