| Biliary extra—drainage be used in biliary obstruction,its purpose is to open drainage,reduce jaundice,support the biliary,and prevent infection. The biliary obstruction caused by cholelithiasis be treated by surgery(remove the stone and drainage biliary) has been confirmed be a effective method.But some patients have some symptoms after the above surgey,these symptoms like pain,distension,indigestion,and so on.The patients who have these symptoms and be treated by routing treatments tell us there is no good effect.At the same time,Traditional Chinese Medicine has been confirmed that it's effect is exactly,if it is used in perioperative of biliary system disorders.Strengthening Spleen and Soothing Liver Therapy derive from Liver-spleen Correlation Theory,and this method has been confirmed that it can promote the restoration of liver function and the recovery of gastrointestinal function,from the point of view Traditional Chinese Medicine and modern medicine.ObjectionTo observe the influence of strengthening spleen and soothing liver for the patient with gall bladder outside drainage skill,and to investigate its possible therapeutic mechanism,to provide a basis for clinical applications.MethodThe study was carried out from 78 cases of biliary tract outside drainage skill hospitalization patients of Guangdong Provincial TCM Hospital surgical department between the November 2007 and March 2008,and randomized controlled method was adopted in this study.They were andomly assigned to a treatment group and a control group.The trearment group with 38 cases were adopted tradition chinese medicine recipt XiaoYaoSan with strengthening spleen and soothing liver as the basic prescription,add or minus according to different patient.Xiao Yao San includes angelica sinensis(oliv.)diels 10 gram, bupleurum chinensis 15 gram,white paeonia lactiflora pall 15 gram,poria cocos (schw.) wolf 20 gram,atractylodes macrocephala koidz.15 gram,mentha haplocalyx briq 5 gram.Control group with 38 cases only adpoted routine treatment,no.tradition chinese medicine.The therapeutic course:beginning from the frist day after surgury,1 weeks is a therapeutic course.Observe some indexes,like recovery time of stomach intestines function,index ALT,AST, TBIL,DBIL,PT,ALB,WBC,and return time of eliminate jaundice.The test data was managed by the statistics software of SPSS11.0.Result1.37 cases of trearment group were invoved into the result anlysis, 1 case was losted.1 weeks is a therapeutic course,the average abdominal pain vanishing time is 12.91±1.43 hours,average anus exhaust time is 17.21±4.02 hours.In control group,36 cases were obervered,2 cases were losted, because the therapeutic schedule have been changed.The average abdominal pain vanishing time is 18.31±1.43 hours,average anus exhaust time is 27.92±3.36 hours.The difference between the treatment group and the control group was significant(P＜0.05).2.In index aspect,beginning from the fifth day after surgury,index ALT, AST,TBIL,DBIL and the reture time of eliminate jaundice of liver function is obvious between treatment group and control group.The 7th day after surgury,ALB,recovery time of stomach intestines function and the juice drainage quantity of gall bladder normal time are different between treatment group and control group.Conclusion1.Strengthening spleen and soothing liver can improve liver function of the patient with biliary tract outside drainage patient surgury.2.Its possible therapeutic mechanism:Strengthening Spleen and Soothing Liver Therapy can promote the recovery of gastrointestinal function by regulate the secretion ofβ-endorphin,and promote the restoration of liver function by improve the liver blood flow,promote liver cell regeneration,and protection of the liver functional reserve.
