Primal Research Of Critical Illness Prognosis Scoring Scale Of Traditional Chinese Medicine

Objective: Through collect the clinical information of Critical illness , by four diagnostic parameters together, combined with differentiation of the eight principles of traditional Chinese medicine to build critical illness scoring system architecture, design the formation of critical illness rating scale of Chinese medicine, according to the scale of clinical critical patients to information collection, and at the same time APACHEⅡscore, Evaluate TCM critical appraisal scale score reliability and validity, make the evaluation relationship of Chinese medicine critical illness rating scale and clinical prognosis of critical illness, and compare with the APACHEⅡscore then to verify the effect, and ultimately the formation of TCM scoring system in critical illness accordance with the requirements of modern medicine surveying and have the TCM clinical characteristics , so to make an objective evaluate the condition of critical illness, prognosis and clinical efficacy . Methods: 358 patients selected from March 2007 to October 2008 in Guangzhou University of Traditional Chinese Medicine First Affiliated Hospital of critical care unit (ICU) , inclusion criteria: (1) consistent with the comprehensive standards of patients admitted to ICU. (2) age of the patients 14-95 years of age. Exclusion criteria: (1) transferred to ICU 24 hours of death or discharge of patients. (2) Serious deficiencies in clinical data were collected.In the first 1h of patients transferred to ICU in critical illness in accordance with the Rating Scale of Chinese medicine, two doctors at the same time independently fill the scale. USE the SPSS for Windowl3. 0 statistical software, through the scale factorα' s appraisal the degree of internal consistency, retest correlation coefficient to retest reliability assessment; analyze the scale of the criterion validity through operation the correlation different scale coefficient between the different field; use LISREL theory and related software EQS for window factor analysis to confirm the structure of study validity scale.Results: A test-retest reliability was found from 200 persons who was in hospital. Intra-class correlation coefficient was 0. 93. Associated 95% confidence interval was 0. 89-0. 96, Split-half reliability was 0. 79. The correlation between serve illness prognosis scoring scale of traditional Chinese medicine and APACHEⅡwas -0.66, non-survivor group compared with survivor group , TCM scores,ages were significantly decreased(p<0. 01). Conclusion: The serve illness prognosis scoring scale of traditional Chinese medicine has good reliability and validity, can evaluate the case fatality in severe illness, it can be applied in the clinical and scientific researches.
