The Observation Of The Clinical Effect About The Treatment Of Qingfei Huatan Huoxue On Decompensated CPHD Patients

Chronic pulmonary heart disease(hereinafter "CPHD") is the disease of the right ventricular hypertrophy and expanding,or even right heart failure due to increased pulmonary vascular resistance and pulmonary hypertension, which caused by chronic bronchi disease,chest disease or pulmonary vascular disease.The disease is a common and frequently-occurring disease in our country.This reseach approach the therapeutic effect about the treatment of Qingfei Huatan Huoxue to the phlegm-heat and congestion type patient of decompensated CPHD use of forward-looking study.Which based on summary of the literature records of ancient Chinese medicine,the research progress of modern Chinese traditional medicine and the research progress of modern medicine about CPHD.Approach the mechanism of action of the treatment of Qingfei Huatan Huoxue to hemorheology based on contrast the level of blood stasis and the hemorheology before and after treatment,thus to provide a scientific basis for treatment of the phlegm-heat and congestion type patient of decompensated CPHD.OBJECTIVE:To observe the therapeutic effect of the Qingfei Huatan Huoxue to decompensated CPHD and the clinical effect on hemorheology.METHODS:60 patients of decompensated CPHD period(sub-light,moderate and severe) with phlegm-heat and congestion were randomly divided into treatment group and control group.The control group use the conventional treatment of western medicine(controlled oxygen therapy,bronchodilation agents,antibiotics, expectorants drugs).The treatment group use the Qingfei Huatan Huoxue decoction of Chinese medicine based on the treatment in the control group. For 10 days.Comparing the clinical symptoms,signs,changes in blood rheology before and after treatment each of the two groups and between the two groups.RESULTS:1.Clinical symptoms and signs:The clinical symptoms and signs of two groups are both relieved after treatment(P＜0.01).The comparing of patient's syndromes and signs(cough,expectoration,gasp,chest tightness,spontaneous perspiration,appetite,abdominal distention,debilitation and pulmonary wheezing sound and other indicators) between the control group and the treatment group were significantly different after treatment(P＜0.05).It shows that both the two groups' therapeutic regimens can relieve the symptoms and signs of patients,,and the treatment group's curative effect is better than the control group's.2.Accumulated points of Ecchymosis:The signs of blood stasis(lips and gingival dark,sublingual varices,jugular varicosity) of the patients are relieved on different levels both of the two groups(P＜0.01 or P＜0.05).while the treatment group is better than the control group in improving the signs of patients with dark lips and gingival,dark purple nails/microcirculation disorde,dark complexion and jugular varicosity(P＜0.05).There are no significant improvement about dark tongue,the pulse,subcutaneous blood spots,appearance of color Doppler ultrasound imaging of pulmonary hypertension or cardiac both of the two groups before and after treatment.3.Hemorrheology peripheral blood:In treatment group,the whole blood viscosity,hematocrit,blood plasma viscosity,erythrocyte sedimentation rate (ESR) and erythrocyte aggregation index improved than before treatment (P＜0.01).The whole blood viscosity,ESR,erythrocyte aggregation index of the control group are also improved after treatment(P＜0.05 or P＜0.01).But the treatment group is superior than the control group in improving the whole blood viscosity,hematocrit,blood plasma viscosity and ESR。CONCLUSION:The treatment Qingfei Huatan Huoxue can improve clinical symptoms and signs, to improve hemodynamic status,promote the rehabilitation of decompensated CPHD patients.
