Clinical And Animal Experimental Study On The Treatment Of AECOPD By Huatan Huoxue Jiangqi Recipe

The first part: Clinical observation on the treatment of acute exacerbation stage of chronic obstructive pulmonary disease(AECOPD)with the method of Huatan Huoxue Jiangqi Recipe.Objective: To explore the effect of Huatan Huoxue Jiangqi Recipe on the clinical symptoms,lung function,and self-assessment test(CAT)scores of patients with AECOPD.Method: 86 patients with AECOPD were randomly divided into treatment group and control group.Both groups were given conventional Western medicine treatments such as anti-infection,oxygen therapy,antiasthmatic,bronchodilators,hormones,etc.The treatment group was treated with Huatan Huoxue Jiangqi Recipe on the basis of this treatment,after the treatment,compare the efficacy of syndromes(total effective rate and total apparent rate)of the two groups of patients,and evaluate the clinical symptom scores,CAT scores and lung function levels(FEV1,FEV1/FVC)of the two groups before and after treatment.Result: After treatment,the therapeutic effect of syndromes in the treatment group was significantly better than that of the control group(P<0.05);the clinical symptom score,CAT score,and lung function index of the patients in the control group and the treatment group were significantly improved compared to before treatment(P<0.05).And the treatment group was significantly better than the control group(P<0.05).Conclusion: Huatan Huoxue Jiangqi Recipe can significantly improve the clinical symptoms of patients,improve the level of lung function in patients with AECOPD,and has a definite clinical effect.It provides clinical evidence for the treatment of AECOPD by the method of Huatan Huoxue Jiangqi.The second part: Experimental study on the treatment of chronic obstructive pulmonary disease(COPD)model rats with Huatan Huoxue Jiangqi Recipe.Objective: To explore the possible mechanism of action of Huatan Huoxue Jiangqi Recipe in the treatment of COPD.Methods: 36 SPF wistar rats were randomly divided into 6 groups,namely blank group,model group,positive control group and large,medium and low dose groups of Huatan Huoxue Jiangqi Recipe.The COPD model rats were established by smoking method combined with intratracheal infusion of lipopolysaccharide(LPS).After the model was successfully established,each Chinese medicine treatment group was given corresponding doses of Chinese medicine by gavage,and the positive control group was given prednisone acetate tablets by gavage.Observe the pathological changes of rat lung tissue;measure the quality of rat lung,spleen,and thymus;detect rat lung function indexes(FEV0.3,FEV0.3/FVC,peak expiratory flow(PEF));calculate rat Organ index;detection of rat alveolar lavage fluid(BALF)and serum tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),endothelin-1(ET-1)levels.Results:(1)The model group showed typical lung tissue pathological changes of COPD.The large and medium dose groups of hormones and Huatan Huoxue Jiangqi Recipe could significantly reduce the pathological changes of rat lung tissue.(2)The lung function level of rats in the model group was significantly decreased(P<0.05);the lung function indexes of the large and medium dose groups of Huatan Huoxue Jiangqi Recipe and the positive control group and the model group were significantly increased(P<0.05));The levels of FEV0.3/FVC in the low-dose group were significantly higher than those in the model group(P<0.05).The lung function index of the high-dose group was higher than that of the middle-dose group and higher than that of the low-dose group(P<0.05).Compared with the positive control group,there was no statistical difference in the pulmonary function indexes of the high-dose group.(3)The lung index of rats in the model group was significantly higher than that of the blank group(P <0.05),and the spleen index was significantly decreased(P <0.05);the lung index of the large-dose Huatan Huoxue Jiangqi Recipe group was significantly lower than that of the model group(P <0.05);The lung index of rats in the high-dose group was not statistically different from that in the positive control group.(4)The serum and BALF levels of TNF-α,IL-1β and ET-1 in the model group were significantly higher than those in the blank group(P<0.05);compared with the model group,serum TNF-α,IL-1β and ET in each dose group-1 level was significantly reduced(P<0.05),The levels of TNF-α,IL-1β and ET-1 in the BALF of rats with large and medium doses were significantly reduced(P<0.05),and the levels of TNF-α and IL-1β in the BALF of rats in the low-dose group were significantly reduced(P<0.05);Comparing each dose group with each other,the high-dose group had the best curative effect;compared with the positive control group,there was no significant difference in the levels of TNF-α and ET-1 in serum and BALF in the high-dose group.Conclusion: The method of resolving phlegm,promoting blood circulation and lowering qi can significantly improve the clinical symptoms and lung function of patients.Its mechanism of action may be to relieve the airway by reducing the levels of TNF-α,IL-1β,ET-1 and other inflammatory factors in serum and alveolar lavage fluid Inflammatory reaction,reduce lung tissue damage,and achieve the therapeutic effect on COPD.
