Clinical Research Of Heart Rate Turbulence In Patients With Chronic Heart Failure

ObjectiveTo study the feature of heart rate turbulence in patients with chronic heart failure. To study the relation of heart rate turbulence and cardiac functional grading.To study the correlation of heart rate turbulence and traditionary index:left ventricular ejection fraction,heart rate variability and so on.MethodsHospitalization patients in this hospital were chosen from May2008 to Mar2009. Based on with and without chronic heart failure they were classified into 2 groups: chronic heart failure(CHF) group(n=58) and control group(n=65).According to NYHA heart functional grading,the numbers of patients in CHF with heart functionâ…¡,heart functionâ…¢,heart functionâ…£are 16,26,16 respectively.The following tests were performed:12-lead surface electrocardiography,echocardiography,chest x-rays, biochemistry,and 24-hour Holter monitoring in the patients of two groups.The patients must meet The following:sinus rhythm,more than 2 ventricular premature complexes (VPCs) in 24-hour Holter monitoring.These indexes were recorded including:HRT, HRV,24-hour mean heart rate,numbers of VPCs,LVEDD,LVEF,QTcd,and so on. Statistical method was used to analyze the all dates.Results1.There was no difference in age,gender and constituent ratio of foundational disease between two groups.There was significant difference in HRT between two groups. Compared with the control group,the value of TO was significantly higher in CHF patients while the value of TS was significantly lower in CHF patients.There was significant difference in positive HRT between two groups.2.There was significant difference in HRV(SDNN SDANN),LVEF,LVEDD, QTcd,and mean heart rate,while there was no difference in ventricular premature complexes of 24 hours between CHF control group. 3.With the increasing of the heart functional grading,the numbers of HRT2 were increase,HRT was significantly different between NYHA heart functional gradingâ…¡andâ…£,â…¢andâ…£.HRT was not significantly different betweenâ…¡andâ…¢.4.The correlation of heart rate turbulence and other indexs in CHF group:TO and heart functional grading,LVEDD were positively correlated;TO and LVEF,SDNN, SDANN,ventricular premature complexes of 24 hours were negatively correlated; There was no significant correlation between TO and QTcd,mean heart rate.TS and LVEF SDANN were positively correlated;TS and heart functional grading,mean heart rate,LVEDD were negatively correlated;There was no significant correlation between TS and SDNN,QTcd,ventricular premature complexes of 24 hours.ConculusionsThe HRT in patients with CHF is decreased.The value of TO is higher and TS is lower in patients with CHF than that in control patients.With the increasing of the heart functional grading,the numbers of HRT2 were increase,HRT can be a marker to reflect heart functional grading and the serious degree of CHF.There were correlation between HRT and heart functional grading,LVEF LVEDD,SDNN and so on,HRT reflect left ventricular function and cardiac autonomic nerve status.
