| PurposeTo observe Xingding injection combined with western medicine treatment of early Diabetic nephropathy(DN) of clinical research, the purpose of the the method of treatment of the clinical effect of DN is to explore better and more effective methods of treatment of DN.By this way,the treatment is not only to improve the quality of life of patients with DN,but also to delay the progress of diabetic renal damage.Methods1.60 patients were selected from the in-patients and out-patients case in HuBei Province Hospital of TCM who were diagnosed Diabetic nephropathy patients.All the patients belonged to Mogensen stages of patients diagnosed with stageⅢ.TCM was blood stasis.They were divided into two groups at random:the treatment group(Xingding injection plus western medicine group)and the control group(western medicine group).Each group had 30 cases.The treatment group and the control group were all treated by routine methods including decreasing blood sugar,controlling blood pressure and adjusting fat.Both the treatment group and the control group used Novolin in order to control blood sugar,we should control the fasting blood glucose during the 6.5 mmol/L and 7.5mmol/L.Blood pressure was controlled by Benazepril hydrochloride tablets and Amlodipine besylate tablets,we should control the blood pressure under 130/80mmHg.If the results of control was unsatisfactory,we could add other drugs such as Betaloc Blood lipids was controlled by Simvastatin tablets.Both the treatment group and the control group during the treatment process did not take Aspirin,dipyridamole,such as anti-platelet aggregation of lipid-lowering drugs and other drugs.The method of the treatment group was on the basis of western medicine plus Xingding injection.The course of the treatment group and the control group was four weeks.The treatment standards which were developed by The National Drug Administration issued in 2002, "Chinese medicine clinical research guiding principles" was used in the clinical research.2.Observation indexes:we observed the symptoms and signs of pre-treatment and past- treatment.And then we wrote the data before and after the treatment.Total cholesterol(TC),triglyceride(TG),blood urea nitrogen(BUN),serum creatinine(Scr),urinary albumin excretion rate(UAER),hemorrheology were examined before and after the treatment.At the same time,we observed the changes of the safety(including Blood-Rt,Urine-Rt,stool-Rt,ECG,and liver function).3.Statistical analysis:All data were processed by statistical software SPSS13.0.Measurement data all adopted t test;count data adopted a chi-square test;the grade data analysis adopted Ridit level data analysis.P＜0.05 was significant difference;P＜0.01 was very significant difference;P＞0.05 was no significant difference. Results1.The treatment group of Xingding injection plus western medicine had eighteen cases of marked effectiveness,six cases of effectiveness,six cases of invalidity.The total effective rate of the treatment group of Xingding injection plus western medicine was 80%;the control group of western medicine had ten cases of marked effectiveness,eight cases of effectiveness,twelve cases of invalidity.The total effective rate of western medicine was 60%.There were significant differences between the treatment group and the control group (P＜0.05).The results of the treatment group were better than the control group.2.The comparison of traditional Chinese medicine symptoms total scores:after the treatment,the traditional Chinese medicine symptoms total scores of the treatment group and the control group had marked reduction in the internal treatment of the same group before the treatment.There was a very significant difference(P＜0.01).The effect of the treatment group was better than the control group(P＜0.01).3.The treatment group and the control group before and after the treatment of blood urea nitrogen(BUN) and serum creatinine(Scr) was no significant difference(P＞0.05).4.There were significant differences in the treatment group including urinary albumin excretion rate(UAER),total cholesterol (TC),triglyceride(TG),and hemorrheology between before and after the treatment(P＜0.01 or P＜0.05).There were significant differences in the control group including urinary albumin excretion rate(UAER),total cholesterol(TC),and hemorrheology between before and after the treatment(P<0.01 or P<0.05).In the control group,there was no significant difference in triglyceride(TG)(P>0.05).The effect between the two groups there were also differences(P<0.01 or P<0.05).The experimental results show that Xingding injection could reduce urinary protein in patients with early DN,lower blood lipids,lowering blood viscosity.It could also improve microcirculation in the body and reduce renal damage of Diabetic in order to slow down the deterioration of renal function.Therefore,the effect of the treatment group was better than the control group.5.In the aspects of safety observation,the treatment group and the control group had no adverse reactions.ConclusionXingding injection plus western medicine treatment of early Diabetic nephropathy which belonged to blood stasis of TCM had exact effect.It not only improved patient's illness of early Diabetic,but also reduced urinary protein and lower blood viscosity,improved lipid metabolism,reduced blood lipids and improved the microcirculation in order to improve the patient's clinical symptoms.Therefore,it could improve the life quality of patients,delaying the occurrence and progress of renal damage in Diabetes.In the clinical application,Xingding injection plus western medicine was one of the better therapies to treat early Diabetic nephropathy(DN).
