| Objective:According to the Yinxingdamo Injection treatment of early diabetic nephropathy(DNⅢ) 30 cases,observe Yinxingdamo Injection early effects of proteinuria in early diabetic nephropathy to explore the mechanism of its effect and the effective way of the injection treatment to early diabetic nephropathy patients.Methods:1.The patients which were consistented with DNⅢ(Mogensen stages) diagnostic criteria and belonged to the TCM differentiation of the spleen and kidney Qi deficiency with blood stasis(infection,acidosis,electrolyte disorders,high blood pressure,blood sugar and so on were under control;during the 18-65 age range;exclusion of other causes of protein uria;not using lipid-lowering,anticoagulant,inhibiting platelet aggregation before the first two weeks of this observation)are randomly divided into treatment group and control group(30 cases of each group).The UAR,age,duration, UAE,Ccr and FPG comparison of the patients in the two groups were not significantly different(P＞0.05) with comparability.2.Control group were mostly given the basical therapeuty,including education,exereise,controlling diet,perfect low protein diet, controlling salt,controlling plasma glucose with insulin or oral durgs to aim at fast plasma glucose(FPG) less than 7.0 mmol/L and plasma glucose of 2 hours after diet less than 10.0mmol/L.Antihypertensive treatment can be used ACEI / ARB or other types of antihypertensive drugs.And the patients in treatment group also extrally received Yinxingdamo injection 30ml combined with0.9%sodium chloride 250ml iv,qd.Both groups were treated for 4 weeks.3.Examine the patients with UAE,TC,TG and other indicators before and after the treatment.At the same time,record changes of the patients with clinical symptoms before and after treatment.And the treatment group compared changes in symptom score with the control group before and after treatment.Results:1.The treatment group and control group both showed a decrease of UAE before and after treatment,with significant differences(P＜0.05).And the urinary protein excretion rate of treatment group decreased significantly more than the control group,with very significant difference which was statistically significant(P＜0.01 ).2.After treatment,triglyceride and total cholesterol both in the treatment group and control group were significantly lower than the triglyceride and total before treatment(p＜0.05).Otherwides,the treatment group significantly decreased total cholesterol levels than the control group,with statistically significant(P＜0.05);Two groups comparing with each other,the decline in the level of triglyceride was no significant difference(P＞0.05). 3.The treatment group Ccr hoisted significantly more than the control group,compared with the control group were significantly different.And the difference was statistically significant(P＜0.05).4.After treatment the value of symptom score of both treatment group and control group were significantly decreased.And there were significant differences(P＜0.01). Besides,The treatment group decreased more than the control group,with the statistically significant difference.It showed that Yinxingdamo Injection can improve the clinical symptoms.And its effect was superior to the control group.5.The total effective rate of treatment group was 86.67%;, the total effective rate of control group was 40%.There were significant differences(P＜0.01).The experimental results showed that:Yinxing Injection treated early DN had better efficacy;and its overall effect was superior to the control group.Conclusion:Yinxingdamo Injection can reduce UAE of the early DN patients;improve renal function of the patients;help patients relieve fatigue,low back pain,limb numbness and other symptoms to improve the life quality of patients.It may be effective in delaying the progress of diabetic nephropathy.
