Ultrasonic Features Of Primary Sclerosing Cholangitis

Objective To study the value of the Color Doppler diagnostic apparatus with high identification and high sensibility in PSC(Primary sclerosing cholangitis).Methods 18 cases of PSC which had been verificated by operation or ERCP(Endoscopic retrograde choledochopancreatography) were reviewed retrospectively.All the patients in hospital came from the contracted department in The First Hospital affiliated to The Shan Xi Medical University or The MeiTan Hospital before February of 2009.To summarize the 2D(2-Dimensional) ultrasonic image.To detect the arteriae hepatica and arterial veins of PSC group and control group by CDFI(Color Doppler Flow Imaging,CDFI).To progress t-test between Corresponding parameter,to analyze the features of arteriae hepatica and arterial veins of Soemmering.To contrast the grouping between US and ERCP.Result 1.2-Dimension 12 hyper-echogenicity of Intrahepatic bile duct,6 tri-intrahepatic bile duct broaden,diameter 0.25-0.35cm;16 hyper-echogenicity of the bile commen duct's walls and 6 stegnosis,diameter 0.30-0.37cm;18 thickening of the choledochous duct's walls,thickness 0.28-0.40cm;12 cholestasis:12 gallbladder,4 bile duct(3 bile clump ).Besides, hepatauxe;splenauxe;abdominal dropsy;lymphadenovarix.2.CDFI PSC group and control group:Volume of blood flow of the arteria hepatica propria(Q_HA) 601.64±87.92 ml/m,P≤0.01;peak value of flow rate(V_MAX) 90.14±17.07 cm/s,P <0.01;resistent index(RI) 0.72±0.06,P≤0.05;volume of blood flow of the arterial veins of Soemmering(Q_PV) 916.90±43.5 ml/m,P>0.05,decreased blood flow of the arterial veins of Soemmering.3.PSC type The 18 patients:intrahepatic bile duct 2,extrahepatic bile duct 3,both intrahepatic bile duct and extrahepatic bile duct 13.US final diagnosis,intrahepatic bile duct 2,extrahepatic bile duct 2,both intrahepatic bile duct and extrahepatic bile duct 10.4 missed diagnosis:1 extrahepatic bile duct,3 both intrahepatic bile duct and extrahepatic bile duct.Conclusions 2-Dimension Besides hepatauxe liver parenchyma,cholecystitis,portal lymph node,hyper-echogenicity of Intrahepatic bile duct and thickening of the choledochous duct's walls are the ultrasonic features of PSC.There is a chance to raise the diagnosis rate,applying the parameters of Arteriae hepatica and arterial veins of Soemmering.It is useful to group the PSC for the hypersound diagnosis.
