Objective:To analyze the clinical characteristics of patients with autoimmune liver disease in Gansu Provincial Hospital,and provide reference for clinical work.Methods:Collected and retrospectively analyzed the characteristics of clinical data of AILD patients who admitted to Gansu Provincial Hospital from January 2008 to January 2018.To analyze the patients’ gender,age,clinical manifestations,serology and pathology,and summarize their characteristics.Results:1.The main groups of AILD are middle-aged and elderly women,the average age was(56.04±9.819)years old.The ratio of males to females was 1:4.8.There was no statistical difference in the average age between the three groups.2.Clinical manifestations: the symptoms of fatigue and anorexia in group AIH were more common in group PBC and PSC.And in group PBC,jaundice was more common than the other two groups.3.Laboratory examination:(1)Patients in AIH group had relatively high levels of ALT and AST.The level of ALP in group PBC and PSC was higher than that in group AIH.The level of GGT in group PBC was significantly higher than that of group AIH and PSC.The TBIL of the three groups increased,and the PSC group was the highest(p<0.05).(2)The AIH group had a higher IgG level than PBC.The level of IgM in group PBC was higher than that of group AIH.(3)In the AIH group,the level of ANA and ASMA is high.In PBC,there are also some specific antibodies,such as AMA and AMA-M2,but the PSC group did not have any specific antibody.4.Accompanying disease:AILD can be combined with other autoimmune diseases.Four patients in AIH group also had sjogren syndrome(6.6%),3 patients with thyroiditis(4.9%),1 patient with rheumatoid arthritis(1.6%).While 1 patient in PBC group had thyroiditis(3.6%),and 1patient had rheumatoid arthritis(3.6%).And 2 patient in the PSC group had ulcerative colitis(20%).5.Histopathology:The histological examination of AIH showed interfacial hepatitis,in sever cases,hepatocytes degeneration and necrosis may occur.The PBC is a granulomatous inflammation in which the small interlobular bile ducts are almost destroyed,and PSC shows the infiltration of inflammatory cells around the bile duct and the proliferation of fibrous tissue.conclusion:1.The main groups of AILD are middle-aged and elderly women;2.Liver pathology is important for the diagnosis of AILD.;3.AIH was the liver parenchyma damage,IgG increased,the common autoantibodies were ANA,ASMA;PBC showed bile duct damage,IgM increased,AMA and AMA-M2 were specific autoantibodies of this disease;PSC also showed bile duct damage.
