| The Lumbar disc herniation(LDH) is a syndrome caused by degeneration of intervetebral disc and rupture of annulus fibrosus,result in bulging out of Nucleus Pulposus that stimulate and compress nerve roots and cauda equine. It is the most common reason for low back pain patients.It was most seen between 20 to 50 years old patients,and the ratio of LDH between male and female was 4-6:1.In tradition Chinese medicine,it was classified into back pain, arthromyodynia and tendinous pain.patients often complain of lumbar and lower limb soreness,and the soreness can be worsen when sitting and walking for a long time,it can cause walking problems,even the disability,and the recurrence rate is high,greatly affect patients' quality of live,In modern medicine,the pathogenesis of LDH was considered closely related to regeneration of intervetebral disc,mechanical compression and the mediator of inflammation.Base on this,the purpose of treatment is to relieve or release the mechanical compression to nerve root,promote subsidise of inflammatory reaction and absorption of inflammatory mediatorNow,the non-operative therapy for LDH have many methods,include drugs, acupuncture,manual therapy,traction and nerve blocking.But in acupuncture, the warm silver needle has more advantages than the common warm needle and acupuncture,for example,because it has better ability of thermal conduction, the warm silver needle can generate stronger thermal effect,and have longer spasmolysis effect for chronic muscle spasm in soft tissue injure.It has shorter course of treatment and lower treatment frequency.Now,the research literature of warm silver needle for LDH is very seldom seen in Chinese or abroad,Based on this,this study will be conducted from the several aspects:1.objective:adopt diameter of 0.4mm warm silver needle to treat LDH,observe the clinical effect of warm silver needle for LDH. 2.1 literature study:studied the relevant name,pathogenesis and intervention methods that related to LDH in the ancient literature,and had a systemic overview for the researching progression of warm needle and warm silver needle in treating LDH in recent years,build a solid theoretical foundation for further study of clinical effect of warm silver needle in treating LDH.2.2.clinical study:equally randomized 60 patients that met the criteria into treatment group and control group,30 patients respectively.All these two groups had basal treatment of firm bed rest,back muscle exercise,out of bed activities under protection of lumbar corset,and regional ventouse,besides, adopt warm needle method,take JiaJi and tenderness point at lumbar-gluteal region as the main pont.Treatment group adopt diameter of 0.4mm warm silver needle,and control group adopt diameter 0.3 stainless steel warm needle. Evaluation with modified japanese orthopedics association(M-JOh) and visual analogue scales(VAS)Pre and post treatment.3.results:3.1 According to study of ancient and recent literature,warm silver needle therapy for LDH has scientific theoretical foundation and satisfactory clinical effects.3.2 There were statistically significant differences in M-JOA and VAS after treatment than pre-treatment(p＜0.05).About effect of VAS:total effect rate is 96.7%in treatment group,the other is 70.0%,the differences between these two groups has statistically significance(P＜0.05);About clinical effect of M-JOA:total effect rate is 90.0%in treatment group,the other is 66.7%,the differences between these two groups has statistically significance(P＜0.05).4.conclusions:There are effect in treating LDH about relieve symptom of pain and clinical effect use two kind of therapy.All of the two group can relieve symptom of pain,improve LDH patient' s daily live and working ability;and the treatment group is better to the other in relieve symptom of pain and clinical effect.
