Objective: To evaluate the safety and efficacy of AcrySof? Toric intraocular lens implantation to correct preexisting corneal astigmatism in cataract surgery. Methods: 36 cataract patients (42 eyes) with corneal astigmatism greater than 0.75D were involved in this study and randomized divided into two groups. Group A with 15 patients (18 eyes) received AcrySof? Toric IOL implantation and group B with 21 patients (24 eyes) received AcrySof? Natural IOL after phacoemulsification. Uncorrected distant visual acuity (UCDVA), best corrected distant visual acuity (BCDVA), corneal astigmatism, residual astigmatism, Toric IOL rotational stability, spectacles independence, patient satisfaction and operative complications were observed.Results: Patients with uncorrected distant visual acuity 0.8 or better accounted for 66.7%(12/18), 72.2%(13/18), 72.2%(13/18) in group A and 33.3%(8/24), 37.5%(9/24), 37.5%(9/24) in group B, respectively. There were significant differences between the two groups 1 week, 1 month and 3 months after operation (x2=4.58,4.97,4.97;P<0.05). There was no statistically significant difference in best corrected distant visual acuity between the two groups (P>0.05). There were no significant differences in preoperative corneal astigmatism and corneal astigmatism 3 months after operation in the two groups respectively(t=-0.37,-0.53;P>0.05). The mean residual astigmatisms were (0.66±0.48)D, (0.74±0.35)D, (0.73±0.34)D in group A and (1.57±1.03)D, (1.42±0.67)D, (1.35±0.53)D in group B, respectively. There were significant differences between the two groups 1 week, 1 month and 3 months postoperatively (t=-3.39,-3.79,-4.02;P<0.05). The mean AcrySof? Toric IOL axis rotation was (4.50±3.03) degrees, (4.80±2.97) degrees, (4.90±2.42) degrees 1 week, 1 month and 3 months postoperatively respectively, with rotation less than 10 degrees in 94.4% of eyes. Spectacles were not necessary in 72.2%(13/18) and 37.5%(9/24)cases in group A and B, respectively. No eye had intraoperative or postoperative complications.Conclusions: The results indicate that phacoemulsification and posterior chamber AcrySof? Toric IOL implantation is an effective option to correct preexisting corneal astigmatism in cataract surgery. The Toric IOL shows good rotational stability. AcrySof? Toric IOL implantation provides a better uncorrected distant visual acuity, spectacles are not required in 72.2% cases and the life quality is improved.
