| Objective:Toric IOL is a new technology in ophthalmology to correct corneal regular astigmatism of cataract patients.The research is to compare UCVA,postoperative residual astigmatism,CS,wavefront aberrations and spectacle independence between Acrysof Toric IOL and Acrysof Natural IOL after 3 month following surgery.The rotational stablility of Acrysof toric IOL after cataract surgery were observed.So we can obtain an objective evaluation of optical qulity of these eyes,rotational stablility and and predictability of Acrysof toric IOL in the correct of corneal astigmatism.Methods:1.sixty-five eyes of sixty-five subjects were enrolled from Tianjin Medical University Eye Center.All patients underwent similar phacoemulsification combined with toric IOLs implantation.We can obtain the SIA using the preoperative and postoperative keratometric data and then obtain a record of the toric IOL model.2.one hundred and seventy eyes of one hundred and thirty-five subjects were enrolled from Tianjin Medical University Eye Center.The preoperative corneal astigmatism was 0.75~2.00 diopter(D) for all the eyes.All patients underwent similar phacoemulsification combined with toric IOLs implantation.Patients with glaucoma,keratitis,fundus diseases and undergoing ocular operation were excluded. 71 eyes of 60 subjects with Acrysof Toric IOL implantation were used in the test group,The Tetraflex accommodating intraocular lens were used in the test group,76eyes of 51 subjects with Acrysof Natural IOL were used in the control group.3.The uncorrected visual acuity(UCVA),best corrected visual acuity(BCVA), postoperative residual astigmatism,toric lens axis,spectacle independence, CS,wavefront aberrations were detected and measured.The postoperative IOL position was assessed.4.Statistical method:the data were analyzed by 11.5 statistical software with methods of independent-sample t test,two-way ANOVA and Pearson correlation, P<0.05 as the standard of significant difference in statistic. Results:1.At 3 months following surgery,96%of eyes in the test group showed the 0.5 or better in UCVA,73%of eyes was 0.8 or better in UCVA and 92%of eyes was 1.0 or better in test group.In control group,70%of eyes showed the 0.5 or better in UCVA, 46%of eyes was 0.8 or better in UCVA and 11%of eyes was 1.0 or better.2.There is statistical significant in the postoperative refractive cylinder between the test group and control group(P<0.05).The mean preoperative corneal astigmatism was(1.53±0.42)D in the test group and(1.50±0.34) D in the control group.In the test group,the mean anticipated residual astigmatism was(0.22±0.11)D in the test group. At land 3 months following surgery,the postoperative refractive cylinder in the test group were(0.21±0.19) D and(0.20±0.19) D,indicatede significant lower than (1.52±0.40) D and(1.53±0.39)D of the control group.There is no statistical significant cylinder between the postoperative refractive cylinder and the anticipated residual astigmatism in the test group(P>0.05)。3.The mean rotation of toric IOLs was(3.74±3.71) degrees after postoperative 1 day.The mean absolute rotation of 3 months after operation was(1.28±1.26) degrees 69 lens showed less than 4 degrees of rotation.One eye occurred more than 10 degrees of rotation after operation.4.At 3 months following surgery,98 percent achieved good distance vision without the use of spectacles in the tset group and 67 percent in the control group.5.There is no statistical significant in the CS between the test group and control group in deferent conditions including in the day time,the day glare time,the night time and the night glare time(P>0.05).6.There is statistical significant in the Zernike C3 and C5 between the test group and control group(P<0.05).Conclusion:1.Toric IOLs showed good rotational stability and predictability in the correct of corneal astigmatism,allow cataract patients the better UCVA,.The combination procedure of phacoemulsification and posterior chamber toric IOL implantation is an effective option for the correct of preexisting corneal astigmatism in cataract surgery.2.Both Toric IOLs and Natrual IOLs showed good CS.There is no statistical significant between them.3.In the Wavescan test,Toric IOLs showed better visual quality than Natrual IOLs. |
