Application On Total Quality Management In Nursing Management

Total quality management (TQM) is originally a concept of manufacturing management. It is first used in the enterprise management, and considerrd as one of effective methods of managing in the management field because a good result is achieved on TQM. One of the content of its core is based on the course of the total quality management. The total quality management thinking applied to the management of hospital care is to focus on the hospital care management, improve work-related activities, and establish a"patient-centered"concept of care, so that the management of all hospital care forms a complete network of services, which will ensure and enhance the quality of care.Hospital care is an important component of the overall curative work. The quality of care reflectes the work level of hospital care and the quality of hospital management, and directly relates to the patient's life and health, impacts the satisfaction degree of the patient on the care, and even plays a vital role in the development of the hospital. In this paper, the construction elements of hospital care quality, and the content and principles of management were systematically analyzed by the studies on the current domestic and foreign hospital care management, which enforce the care TQM through to aim directly at some questions in the care management and to use the theory and methods of the business TQM.The nursing staff and patients were choosed as the investigating subjects in the second Hospital of Jilin University during March, 2006– April, 2008. The quality of care was evaluated by using random sampling method for the care unit of some sections in the hospital to research the related issues of enforcing TQM in the care and guide the quality of care management practice by the implementation of TQM in order to further deepen the connotation of care. Dozens of the nursing care management standards and assessment rules were revised and consummated according to the requirements of hospital management at different levels and the specification of related management of care work, which perfected the position and responsibility of the nursing staff at all levels, and the evaluation criteria of nurse behave, the overall care effectiveness, the care quality and care quality inspection. The care indes were scalarized on the basis of the standards, which revised the conventional care specialist, specified a variety of standardized care forms and the technical operation of basic care, unitied normative care writing instrument and manual recording standards of the nurse head, and achieved the systematization, specification and standardization. The quality of care unit nurses, ward management, special units, technical operations, care instrument, basic care and grade care were examined and evaluated with random spot-checks and the satisfaction degree of the patient on the professional image and quality of care staffs to determine the care effectiveness. At the same time, we also continued to observe and inspect the study subjects—the image of nursing services, quality of care, and so on during the study.The results of this research showed that the qualities of basic nursing, grade care, writing document in nursing, the diathesis of nursing staffs, the basic theory skills of nurses, the work of nurse head, the ward management, the special unit management in clinical divisions and whole nursing care in hospital all increased after enforcing TQM, so the satisfaction deree of patients increased, a virtuous circle of TQM formed, and the the efficiency of nursing management improved.TQM is a continuous process, a more scientific means of promoting quality, an advanced method of constantly ensuring a high standard of care quality and an eternal goal of quality management. TQM has not the end, only demands the continuous improvement and constant innovation with the ultimate aim of satisfaction degree for enhancing the patients and other relevant aspects. TQM concerns the entire process of the quality supervision, which emphasizes constantly locating higher standard on the basis of the original quality management, so that the quality of care is always in a benign cycle in orbit. TQM continued to improve the quality of care is an effective management method for adapting needs of the times, patient's request and the improve of care quality. The care quality elevated by analyzing the dissatisfaction deree of patients and finding the problem-solving causes, improving the care quality in order to prevent the occurrence of the problem without the inspection and remedies after the event. Therefore, the care quality only improves permanently and fundamentally through the quality control before the event. In the process of raising the care quality, highly thinking of the leader is the key. Only the leader realizes really to the importance and urgency for quality management in establishing and improving the quality of the system with providing adequate resources, leading participation in a variety of quality control, and going to work with an example according to the document requisition of the quality system, the nurse head and first-line nurses can unity the recognition and surefooted work to improve the management quality as a major event, self-consciously assume the responsibility in the quality control activities, and commendably do the care quality work.The above-mentioned series of studies provide the shallow draw and reference for the care manager. In this paper, the novelty intends in the TQM concept and method used in the hospital care management. But this study was not enough in-depth to explore the theoretical system, not enough broad to analyzing the field of vision, and not enough clear to taking strategy for the problems arising in enforcing process. The quality of care indicators is sometimes difficult to predict as the patients of nursing service and the nursing staffs of nursing service are different in a thousand ways, and providing service quality suffers the influence of many subjective and objective factors. At the same time, the care management in the hospitals of different levels has respective different characteristics and specific ways, so adopting care management of any way is depending on the specific circumstances of each hospital. However, the care TQM is a trend and worthy of further study as more and more people concern about the hospital quality construction, and the study on the hospital care quality management theory and evaluation system develops overall, scientifically, rationally and deeply to increase the satisfaction degree of the patients and promote the construction of monitoring system.
