ObjectiveThis study aims to investigate the level of TNF-αand SP in GCF of the abutment teeth with healthy periodontal condition and adult periodontitis after PFM crown placement by radioimmunoassay,and also inquire into the influence of crown placement and different crown margins on abutment teeth of healthy periodontal condition and adult periodontitis from microcosmic level.The purpose of the study was also to make a tentative probe into the influence of PFM crown placement on the advanced mechanism of periodontal inflammatory condition of adult periodontitis and provided with experimental basis to optimizing prosthodontic device of abutment teeth with adult periodontitis.Methods1.The study population consisted of 56 periodontally healthy patients and 41 patients with mild chronic adult periodontitis in whom 43 maxillary and 57 mandibular molars which would be restored by PFM.The total of the molar are one of the two abutment teeth of fixed partial dentures of 3 units.They were grouped into three groups at random by order at first visit,including supra-gingival location of the crown margin, sub-gingival one,and with the margin at the gingiva.And the teeth with the same name as the test teeth with periodontitis on the opposed side of the same jaw bone were served as controls.The clinical parameters of periodontal conditions,including gingival index(GI),plaque index(PLI),probing depth(PD),attachment loss(AL) were recorded and the GCF of teeth were collected at the time before teeth preparation and at 6-month-follow up after crown placement respectively.2.Examine the level of TNF-α,SP by radioimmunoassay,Statistical analysisThe data were expressed as Mean±SD,differences between the data of the same group before and after treatment were analyzed by t test and mutual comparison of the three groups were analyzed by One-way ANOVA.The correlation of the level of TNF-αand SP was analyzed by Spearman correlation analysis.Results1.Significant differences(P<0.05)were noted for the level of TNF-αand SP in the GCF of abutment teeth between 6 months after crown insertion and pre-restoration in the group of teeth with healthy periodontal condition,as well as between 6 months after crown placement and pre-restoration or control teeth in the group of teeth with adult periodontitis.2.There is no statistically difference in the levels of TNF-αand SP in GCF of group of supra-gingiva,at gingiva and sub-gingiva in the groups of teeth with healthy periodontal condition when they were compared each other(P>0.05).3.The level of TNF-αand SP in GCF of teeth of group of sub-gingiva showed statistically higher than that of group of supra -gingiva and at the gingiva in the group of teeth with periodontitis when 6 months follow-up after crown insertion(P<0.05), The level of TNF-αand SP in GCF of teeth of group of at the gingiva showed higher than that of group of supra -gingiva(P>0.05).4.The positive correlation was found between the level of TNF-αand SP in GCF of teeth with healthy periodontal condition and periodontitis(P<0.05).Conclusions1.Crown placement was identified as risk factors which results in the damage of periodontal tissues of abutment teeth.The initial periodontal condition of the abutment teeth is a critical factor which decides the destructive activity. 2.Crown gingival margin placement of at gingiva and sub-gingiva may aggravates the inflammation of periodontal tissues of restored teeth with periodontitis,margin placement of supra-gingiva is one form which have a slightest affect on the periodontal tissues of restored teeth with periodontitis,3.There is no significant difference in the influences of three forms of crown margin placement on the healthy periodontal condition of 6 month restored teeth when they were compared each other(P>0.05),the long-term results remain to be investigated.4.Monitoring the level of TNF-αand SP in the GCF may provide with valuable information for judging inflammatory and.immune status of periodontal tissues.
