Objective: By analyzing the expression of HLA-ABC and HLA-DR in the spleen of patients with cirrhosis and protal hypertension, and the relationship between the expression and some correlated clinical parameters, studied the possible mechanism of the splenic immune function change in those patients.Methods: The expression of HLA-ABC and HLA-DR in the spleen of 45 patients with cirrhosis and protal hypertension and 15 traumatic rupture patients was tested by the Envision immunohistochemistry method. A correlated analysis was conducted between the result of the expression and some clinical parameters.Results: The positive ratios of HLA-ABC and HLA-DR expression were (31.04±8.21)% and (7.57±2.57)% in the cirrhotic spleen respectively, while (46.54±6.14)% and(13.46±1.89)% in the control group, which showed significant differences between groups (P<0.01). The expression of HLA-ABC and HLA-DR was decreased associated with the aggravation of the cirrhotic splenic levels and Child-pugh types of liver function. The group of cirrhotic splenic levels I had a significant difference with levels II and III (P<0.05), but had no significant difference with the control group (P>0.05). The group of Child-pugh types A had a significant difference with types B and C (P<0.05), but had no significant difference with the control group (P>0.05). The expression of HLA-ABC and HLA-DR had association with cirrhotic splenic levels and Child-pugh types of liver function (P<0.01), but had no association with age, sex, the frequency of upper gastrointestinal bleeding, and the level of esophagogastric varices (P>0.05).Conclusions: The positive ratio of HLA-ABC and HLA-DR expression in the spleen with cirrhotic protal hypertension was obviously lower than it is in the control group. It may be one of the reasons that the splenic immune function of patients with with cirrhosis and protal hypertension was lower than normal. The result of the cirrhotic splenic levels and Child-pugh types of liver function may helpful for the evaluation of the cirrhotic splenic immune function of those patients.
