Objective:The purpose of this article is to evaluate the value of urinary dynamics in the diagnosis, differential diagnosis , disease grades , complications , prognosis , and curative effect on BPH patients accompany with diabetic mellitus, investigate the clinical significance of urodynamic studies .Materials and Methods:A total of one hundred and six BPH patients with lower urinary tract symptoms. The mean age of the patients was 53-92 years. Mean duration of voiding symptoms was 3-12 years. These patients were divided into the two following groups: Group A consisted of 59 BPH patients and Group B consisted of 47 BPH patients with diabetes mellitus. Both groups were evaluated for the international prostate symptom score(I-PSS), urine flow rate, and residual urine volume All cases underwent urodynamic examination. Correlation analysis was conducted to determine the correlation among the patients. The urodynamic result in different groups were compared.Results:Urodynamic studies showed that group A : Qmax(5.1±2.3)ml/s, the residual urine volume is (156±92) ml, IPSS( 27.7±5.4), 34 patients(72.3%) had detrusor instability of bladders, 14 patients (29.8%) had low compliance bladders; 21 patients (44.7%) had damaged detrusor function. Three months after operation , recheck ination showed unsatisfactory results in 8 patients (17.0%). Of them, 5 patients still had dysuria, which was due to remnant obstruction and complicated bladder neck contracture ;the other 3 patients hadurinary incontinence, with damaged external sphincter and severe D I and low compliance bladders. In control group B: Qmax(7.7±4.3)ml/s, the residual urine volume is (159±91) ml, I-PSS(19.1±4.3), 35 patients(59.3%) had detrusor instability of bladders, 10 patients (16.9%) had low compliance bladders; 16 patients (27.1%) had damaged detrusor function. Three months after operation, recheck ination showed unsatisfactory results in 3 patients (5.1%). To compare two groups:The IPSS in group A is higher than B, while the Qmax is lower than B.The unsatisfactory results of A is higher than B. There were statistically significant differences between the two groups, But there were no statistically significant differences between the two groups in residual urine volume, (P >0. 05).The urodynamics result: There were statistically significant differences between the two groups in detrusor instability of bladder, low compliance bladder and damaged detrusor function, ( P < 0.05).Conclusions:The influence of diabetic mellitus to the function of bladder in BPH patients is significance, and we should not neglect it. The best program should be to control blood glucose ideally .It must be improve the postoperative curative effect and decrease postoperative complications in BPH patients accompany with diabetic mellitus. The urodynamics has an important referenced value in BPH patients accompany with diabetic mellitus.
