Objective To observe Urodynamic findings and analysis partial clinical data,to investigate the clinical significance of urodynamic changes in benign prostatic hyperplasia(BPH)patients with hypertension.Methods A total of 131 BPH patients were enrolled.The Urodynamic findings,effects of International Prostate Symptom Score(IPSS),Quality of Life(QoL)and the Prostate Volume(PV)measured by transabdominal ultrasound were assessed.According to the diagnostic criteria of BPH and hypertension,patients with BPH were divided into two groups: BPH group without hypertension(G0,n=45)and BPH with hypertension group(G,n=86).According to the blood pressure stages of BPH patients,the BPH combined with hypertension group was further divided into grade 1 hypertensives group(G1 group),grade 2 hypertensives group(G2 group)and grade 3 hypertensives group(G3 group).According to the patients' Systolic Blood Pressure(SBP)and Diastolic Blood Pressure(DBP)levels,they were divided into high SBP group(GS group,n=13)and high DBP group(GD group,n=33).According to the length of history of hypertension,patients were divided into two groups: The history of hypertension within 5 years group(C1 group,n=35)and the history of hypertension more than 5 years group(C2 group,n=51).The urodynamic characteristics of patients with BPH suffered from hypertension were analyzed by comparing the clinical data and the urodynamic parameters of patients in each group.Results The average age of the patients was 70.11 years old.Among all the enrolled BPH patients,45 patients with normal blood pressure and 86 patients had hypertension.1.To compare BPH patients with hypertension and BPH patients with normal blood pressure.The patients in G0 group had higher Pdet.Qmax,Ppeak,Pave,Qmax than those in G Group,while had lower PV,IPSS,FDV,MCC,VRU than those in G Group.And the proportion of patients with reduced bladder compliance,decreased detrusor stability,and lower urinary tract obstruction in G group were significantly higher than those in the G0 group(P<0.05).2.To compare BPH patients with different blood pressure stages.The patients in G0 group had higher Qmax,Pdet.Qmax than those in G2 Group,the patients in G0 group had higher Ppeak than those in G1 and G2 Group,the patients in G0 group had higher Pave than those in G2 Group,the patients in G0 and G1 group had higher Pv than those in G2 Group.The patients in G0 group had higher IPSS than those in G2 and G3 Group,the patients in G1 group had lower IPSS than those in G2 Group,the patients in G0 group had lower FDV than those in G1,G2 and G3 Group,the patients in G0 group had lower MCC than those in G2 Group,the patients in G2 group had higher VRU than those in G1 Group(P<0.05).3.To compare BPH patients with high SBP and high DBP,the patients in G0 group had higher Ppeak and Pavethan those in GD Group,had smaller PV than that in GDGroup,had lower IPSS,FDV,MCC,VRU than those in GS Group(P<0.05).The patients in G0 group had higherQmax.Pdet,Qmax,Ppeak,Pavethan those in Gs Group,had smaller PV and lower IPSS,FDV,MCC,VRU than those in Gs Group(P<0.05).4.To compare BPH patients with different length of history of hypertension,the patients in C1 group had higher Qmax,Pdet.Qmax than those in C2 Group,while and lower FDV,MCC,VRU than those in C2 Group.Conclusions 1.The Qmax in BPH patients with hypertension is significantly reduced,moreover,the higher degree and the longer history of hypertension,the more significant reduction of Qmax.2.The bladder sensation,bladder compliance and detrusor stability in BPH patients with hypertension are decreased significantly.3.BPH patients with hypertension have obvious prostatic hyperplasia,increased bladder volume and increased residual urine volume.
