The Clinical Study Of The Treatment Of Different Level Lumbar Disc Herniation By Oblique-pulling Orientation Manipulation

Significance: The Lumbar disc herniation(LDH)is one of most commonly orthopedical disease Lumbar disc herniation is a diagnosis given by western medicine, however, this name does not exist in Traditional Chinese medicine. this name does not exist in Traditional Chinese Medicine. Instead, this disease is categorized to lumbar and leg pain. Tuina manipulation is one of the master-pieces of Traditional Chinese Medicine,it has a long history.It is thought that the main benefits of Tuina are to dredge meridians,promote blood circulation, adjust Zangfu function and lubricate the joint. Moreover, it helps build up ability of self-resistance to disease and as a result of fact it willl prevent and heal disease.Professor Lin yingqiang of Massage Department at GuangDong Provincal Tradition Chinese Medical Hospital has thirty-year clinical experiences in orthopedics who has concluded a unique type of Lin's locating manipulation according to the physiological and pathological feature of LDH. This manipu lation has been put into use for twenty years and has received very good therapeutic effects. In order to discuss and study the measure standard of curative effect evaluation regarding Oblique-pulling Orientation Rotatory Manipulation(OORM) which is used to treat different level LDH, and make it scientize and standardize, therefore, we have designed this investigation.Objective: To study the clinical availability of OORM by clinical randomny-contrasting-experiments. According to observe the Chinese Oswestry disability index (CODI) with LDH, we could anaylize the difference of on the patients' function improvments who were treated by OORM with different level LDH.Methods: In this study, we chose and treated 240 patients with LDH from Moxibustion Dept. f Guangdong Province Hospital of Chinese Traditional Medi cine between June 2006 and March 2008.Those who met the inclusion criteria were randomly divided into two groups: The treatment group were performed OORM for a course of treatment,and meantime the control group were performed general rotatory manipulation. All the patients were assessed once before being treated using function obstacle CODI, and were tested for adverse reaction to this procedure. Then they were assessed by CODI again after one period of treatment and finally we have the datas statistical analysis.Results: 1. The result showed that the difference of post and prior-treatment CODI between two groups was significantly different (P<0.01); After the treamen, there were significant differences between the CODI and the cured and markedly effective rates of the two groups (P<0.01) ;2. There was no significant statist tical difference between the prior-treatment CODI of the different level groups (P>0.05). It means that the two groups could be compared with each other. 3. After a course of treatment with different methods, we detected the CODI one more time. It indicated that there were significant differences in the different level groups detected post and prior-treatment (P<0.01) and also in the same two level groups detected post-treatment (P<0.01) .However compaired with CODI of the different level groups after the treatment: there was no significant statistical difference between L4/5 and L5/S1 treatment group (P>0.05) , the difference between L4/5 and L3/4 treatment group was significantly different (P<0.01) and the same result to the difference between L5/S1 and L3/4 treatment group (P<0.01).Conclusions: 1. The two kinds of manipulations both make obvious improvements in patients'CODI function with LDH. But compared with these two rotation manipulations,OORM might be a better treatment means for LDH. 2. After a course of treatment, the patients with different level LDH of these two groups have some differrent improvements in function obstacle. However,the improvements of L3/4,L4/5,L5/S1 treatment group are superior to the same level of control group. It clearly showed that OORM is more benefitful to the CODI than general remedy in the treatment of LDH. Forever, according to the deeply clinical research between these cases finding out: the improvements of CODI are much better in L4/5,L5/S1 the two level LDH than those in L3/4 level LDH. 3. After this reseach, we come to a opinion on that Lin's OORM is simple, quick and effective in the treatment of LDH. Therefor Lin's SORM should be generalized in the clinic.
