Background:Neck disorders remain a acommon problem in modern industrialized countries. The origin and exact pathophysiologic mechanisms of chronic neck pain is obscure. Cerv-type cervical syndrome is initial phase and found condition of all the neck pain. Becase of tiny physical sign and local symptom, pathogenetic condition is easy to be ignored, and the patients are not paid more attention; Now there is no treatment to aim directly at the chronic neck pain abroad, seriously in China.Comparied to the healthy people, cervical muscle of neck pain is easy to get tired, especially in posterior muscle group. Qishi confirmed that disequilibrium of muscle strength could induce disc degeneration by empirical study. " Pay equal attention to bones and muscles,dynamic and static combination" is an important therapy principle of neck pain in traditional Chinese medicine. Active physical training is usually recommended for patients with chronic neck pain; however, its efficacy has not been demonstrated in randomized studies in China. Objective:Compared to the effect betweem manipulation added neck isometric exercises and single manipulation treatment to document the predominance of isometric neck strength training. Materials and Methods:Manipulation treatment as a parallel control group, and to select 95 subjects from 2005 to 2008 according with diagnostic criteria of Cerv-type cervical syndrome. All the patients with chronic neck pain were randomized into manipulation group and isometric neck strength training group, using Visual Analog Scale,Vernon Cervical Disability Index and the muscle strength to evaluate the efficacy. Result:The 95 subjects were followed up. No one in the treatment group aggravated and one case in control group develop into cervicalspondylotic radiculopathy, another one change into sympathetic cervical spondylosis. The frequency of serious neck pain of manipulation group and isometric neck strength training group is 6. 65±2. 62 and 2. 27±1. 5 times in three months; Visual Analog Scale,Vernon Cervical Disability Index of two groups after treatment had remarkable significance(P<0. 05), and the muscle strength of the treatment group after treatment had remarkable significance(P<0. 05) ; After treatment comparing to the control group, Visual Analog Scale and the muscle strength in treatment group had remarkable significance (P<0. 05) ; Three months later comparing to the control group, Visual Analog Scale,Vernon Cervical Disability Index and the muscle strength in treatment group, remarkable significance (P<0. 05); In the control group, the muscle strength in three periods, no remarkable significance (P>0. 05). The muscle strength was negative associated with Visual Analog Scale and Vernon Cervical Disability Index before treatment. Then after the treatment and 3 months later also negative associated with Vernon Cervical Disability Index, but no correlation with VAS. Safe evaluation, there were four cases occuring aching pain among neck and nape and upper limb and six cases having red seam, vanished after training. And then recovery normal exercises; There was no complaints in control group. Bone fracture,luxation,apopsychia and other adverse effects did not happened in both groups. Conclusions:Comparing to manipulation group, Isometric neck strength training can improve the neck pain and muscle force, reduce the recurrence of pain and enhance cervical function, having superiority short-term efficacy.
