Background and objectivesSimple obesity is a common disease in children,which is simply due to over-nutrition and calorie intake more than consumption resulting in the accumulation of excess fat in body.The measurement of fat tissue is an important basis in diagnosing simple obesity.Leptin is the obesity gene mainly produced in body fat tissue.DXA is a method of measuring body fat content directly.This project aims at investigating the clinical applied value of Leptin and DXA in measuring body fat and changes in body fat in children with simple obesity.MethodsThe study included 248 healthy pre-pubertal children,which had 159 normal children and 89 children with simple obesity.All of the children were measured the total body fat by dual energy x-ray absorptionmetry(DXA)and detected the serum Leptin by ELSIA.Results1.In the normal group,the Leptin level in boys(3.55±2.66ng/ml)was significant lower than girls(7.19±6.88 ng/ml)(F=20.95,P<0.01).2.In the obesity group,the Leptin level had no significant difference between boys(31.37±18.62ng/ml)and girs(30.51±14.79ng/ml)(F=0.04,P>0.05).3.In the normal group,the body composition by DXA,arms fat content in boys are much lower than girls(256.30±158.98g Vs 354.29±226.6g,F=11.65,P<0.01);legs fat content in boys are much lower than girls(1395.56±649.87g Vs 1833.84±936.83g, F=16.92,P<0.01);trunk fat content in boys are much lower than girls(1692.04±810.50g Vs 2256.03±1262.62g,F=15.67,P<0.01);total fat content in boys are much lower than girls(3725.94±1646.92g Vs 4867.18±2492.06g,F=16.07, P<0.01);total fat percent in boys are much lower than girls(16.63±4.26%Vs 21.42±5.54%,F=39.26,P<0.01);trunk fat percent had no significant difference between boys and girls(45.05±3.95%Vs 45.69±3.10%,F=1.45,P>0.05).4.In the obesity group,the body composition by DXA,arms fat contents had no significant difference between boys and girls(1674.44±856.91 gVs 1447.74±553.18g, F=1.4,P>0.05);legs fat contents had no significant difference between boys and girls(6184.23±2626.87g Vs 5796.73±1913.45g,F=0.47,P>0.05);trunk fat contents had no significant difference between boys and girls(7916.54±3577.25g Vs 7341.44±2459.86g,F=0.58,P>0.05);total fat contents had no significant difference between boys and girls(16551.41±7111.17g Vs 15317.92±5007.26g,F=0.66,P>0.05); trunk fat percent had no significant difference between boys and girls(47.49±2.80% Vs 47.55±1.95%,F=0.01,P>0.05);total fat percent had no significant difference between boys and girls(37.64±7.85%Vs 39.07±6.74%,F=0.54,P>0.05).5.In the normal group,the BMD by DXA had no significant difference between boys and girls(0.81±0.06g/cm~2 Vs 0.80±0.06g/cm~2,F=4.43,P>0.05).6.In the obesity group,BMD by DXA had no significant difference between boys and girls(0.89±0.07g/cm~2 Vs 0.87±0.06g/cm~2,F=2.24,P>0.05).7.BMD had significant difference between obesity group and normal group (P<0.01).8.The contents of Leptin in all children highly positive correlated with arms fat contents,legs fat contents,trunks fat contents,total fat contents and total fat percent(r=0.8962,P<0.01;0.8806,P<0.01;0.8830,P<0.01;0.8889,P<0.01;0.8718, P<0.01,respectively).The contents of Leptin lowly positive correlated with trunks fat percent(r=0.3392,P<0.01).Conclusions1.The serum level of Leptin in obesity children were significantly increased than that in normal children,but had no sexual difference.It suggested that Leptin was an objective index to reflect body fat contents.2.The contents of body fat and total fat percent in obesity children were significantly increased than that in normal children.The contents of body fat and total fat percent in obesity children had no sexual difference.It suggested that DXA was an objective and accurate method to detect the contents of body fat.3.The body fat of prepubertal children were all in uniform distribution.4.The BMD in obesity children were increased than that in normal children.5.The serum level of Leptin was positive correlated with the contents of body fat. It suggested that the serum level of Leptin and DXA could be served as the diagnosis evidence and evaluation index for simple obesity in childhood.
