Objective:To discuss the relationship between serum uric acid level and coronary artery lesion severity,coronary heart disease(CHD)type,systolic and diastolic heart function,cardiovascular events and to evaluate whether serum uric acid is an independent risk factor of CHD or not.Methods:866 cases with coronary angiography hospitalized in the Second Hospital of Tianjin Medical University cardiologist from April,2005 to April,2007 were collected.The clinical indicators of gender,age,medical history,laboratory results, coronary artery lesion severity,echocardiography results and cardiovascular events during hospitalization of all the patients were recorded.According to the results of coronary angiography,866 cases were divided into CHD group(706 cases)and non-CHD group(160 cases).Then compared the difference of the clinical indicators between the two groups.According to serum uric acid level,CHD group was divided into high uric acid subgroup(138 cases)and normal uric acid subgroup(568 cases). Then compared coronary artery lesion severity,CHD type,systolic and diastolic heart function and cardiovascular events between subgroups.Besides CHD group was divided into SAP,UAP and AMI subgroup according to the type of CHD and divided into the single-vessel,double-vessel and multiple-vessel subgroup according to the count of vascular lesions.And then serum uric acid level was compared among three subgroups,respectively.Results:1 Serum uric acid level in CHD group was significantly higher than non-CHD group[(328.02±91.97)μmol/L vs(293.49±80.65)μmol/L],P<0.05.2 The detection rate of hyperuricemia was 17.8%,including 10.9%of men and 6.9%of women.The prevalence rate of hyperuricemia in CHD group was significantly higher than non-CHD group[19.5%vs10.0%],P<0.05.3 Serum uric acid level were positively correlated with blood urea nitrogen(BUN),serum creatinine(Scr), triglyceride(TG)and negatively correlated with fasting plasma glucose(FPG),high density lipoprotein cholesterol(HDL-C).The correlation coefficient was respectively 0.258,0.I54,0.140,0.231,0.435,0.156,-0.140,-0.151,P<0.05.4 The level of serum uric acid was respectively(327.19±96.03)μmol/L,(328.45±94.05)μmol/L, (333.03±95.10)μmol/L in single-vessel subgroup,double-vessel subgroup and multiple-vessel subgroup.There was an increasing trend of serum uric acid level in single-vessel subgroup,double-vessel subgroup and multiple-vessel subgroup.The level of serum uric acid in three subgroups was higher than non-CHD group.And the difference was not significant,P>0.05.The integration of coronary artery was respectively(4.47±1.64),(7.65±1.79),(12.04±7.06).With the number of vascular lesions increased gradually,the integration increased.The difference was significant, P<0.01.5 The level of serum uric acid in SAP subgroup,UAP subgroup and AMI subgroup was respectively(322.41±84.91)μmol/L,(329.08±97.36)μmol/L, (354.98±102.50)μmol/L.There was an increasing trend of serum uric acid level in SAP subgroup UAP subgroup and AMI subgroup.The level of serum uric acid in three subgroup was significantly higher than non-CHD[(293.49±80.65)μmol/L], P<0.01.The difference in three subgroup was not significant,P>0.05.6 The ejection fraction was respectively(53.91±16.49)%,(54.35±12.16)%,(60.71±9.95)%in high uric acid subgroup,normal uric acid subgroup and non-CHD group.There was an increasing trend in three groups.The ejection fraction of high uric acid subgroup and normal uric acid subgroup was lower than non-CHD group,P<0.05.7.The prevalence rate of total cardiovascular event and arrhythmia in high uric acid subgroup was significantly higher than normal uric acid subgroup[(59.4%vs 18.7% and 44.9%vs 7.7%)],P<0.05.8 With binary logistic regression analysis to CHD,age, smoking history,white blood cell count(WBC)and serum uric acid can be taken as an independent risk factor of CHD.Conclusion:Serum uric acid level is closely related to the occurrence,development, pathological changes and prognosis of coronary heart disease and may be an independent risk factor of CHD.It can be used for finding timely and earlier the dangerous crowd of CHD in the clinical.
