Background ; Diabetic cystopathy (DCP) is a frequently common chronic complication of diabetes. About 40%~80% of DM patients suffer from DCP. It gradually develops without symptoms and is often neglected until urinary retention and urinary tract infection, even renal failure happened. As a result, its prognosis is poor. In the past, we judged the function of DCP patients' urinary bladder according to the chief complaint of patients about the symptom of dysuresia,absense of objective diagnostic results. The prevension of progression of disease and choice of therapy methods were limited .The application of urodynamics detection in determining the changes of urodynamics index is significant for differential diagnosis of urinating functional disturbance caused by cystipathy. Urodynamics test examination is the most important means in objective evaluation of bladder function. Urodynamics index can reflect the changes of function of diabetic bladder, then can also be used to judge whether diabetic patients suffer from DCP. And the recent studies show that DCP can be diagnosed early if urodynamics test is taken, Partial course of DCP disease is reversible If DCP can be diagnosed and treated actively at an early stage. At present, The diagnosis is mostly taken by urinologist with a invasive urodynamics test, to exam patients with obvious dysuresia. It is applied in DCP patients who have had urinary retention and needed to determine the bladder detrusor pressure to decide whether urological operation should be taken. It is hard to take the test for DCP patients without obvious dysuresia, with the chance of early diagnosis lost.The noninvasion urodynamic examination is a latest concept, with the advantage of examing urodynamics index noninvasively, then with early diagnose for DCP. The DCP diasease is a crossing subjects. It is not yet reported that noninvasion urodynamic was used to make early diagnosis for DCP so far.Objective: To evaluate the value of detection on the early diagnosis of DCP with the technology of noninvasion urodynamics to the patients of DM which no urinary clinical symptom and explore early prevent and therapy.Methods: 70 patients with type 2 diabetics mellitus(DM) and 30 normal control people were examined with the technology of noninvasion urodynamics. Bases on their disease course: less or more than 5 years, the DM patients were divided into two groups . Maximum flow rate, average flow rate, the volume of first bladder sensation, residual urine volume were measured by using dynamic technology.Results: Among the 70 DM patients, 34 were detected bladder residual urine. So the DCP detection rate were 48.6%. In the patients with DCP, the average residual urine volume was 30.1±27.1ml (7~139ml). While there were no residual urine in the normal control group. Compared with the normal control group, Maximum flow rate and average flow rate were decreased in all patients with DM and those patients with DCP(P < 0.01). Follow the course of disease, the patients with more than five years disease control, their Maximum flow rate and average flow rate were even lower.Conclusions: (1)Maximum flow rate decrease and bladder residual urine were detected with the technology of noninvasion urodynamics may be used in early detection and early diagnosis of DCP. (2) The apply with the technology of noninvasion urodynamics is simple. The characters of the technology of noninvasion urodynamics is widely used and early detection on DCP. It can be early detection and early diagnosis of DCP.
