Ultrastructural Study On Endometrial-myometrial Junction Of Adenomyosis And The Effects Of Mifepristone

Objective To study the relationship between the ultrastructural change of endometrial- myometrial junction and adenomyosis. Meanwhile, To observe the effects of mifepristone on ultrastructure of endometrial-myometrial junction of adenomyosis. To approach the mechanism of action of mifepristone.Methods After hysterectomy, cutting the endometrial-myometrial junction at the basal part of body of uterus to make the ultrathin section. Using transmission electron microscope to observe the ultrastructure of endometrial-myometrial junction from 8 patients with adenomyosis and 6 patients with adenomyosis took mifepristone 5mg/d orally in 4 weeks(mifepristone group), and 8 controls without adenomyosis. To unify morphology and pathophysiology.Results (1) Compared to control group, In adenomyosis group, The intercellular spaces between the glandular epithelium became wide , desmosome plaque were seen. microvilli on the surface of glandular epithelium were close. in the cytoplasm, mitochondria were enlarged and active, There was no apoptosis; Smooth muscle cells laid irregularly, the intercellular spaces became wide, Their normal constitution distorted, Organelle growed downwards, mitochondria were vacuolization, rough endoplasmic reticulum were dilated, the heterochromatin of nucleus were increacred, appearing to gather fringe; mast cells growed in number, they were different in sizes and morphous. in the cytoplasm, there were many electron-dense particles. collagen fiber was increased nearby , mast cells were closed together Smooth muscle cells.(2) Compared to adenomyosis group, In mifepristone group, glandular epithelium were scarce, cell membrane were incomplete thread, microvilli on the surface of glandular epithelium fell off. Organelle growed downwards in the cytoplasm, mitochondria were vacuolization. The nucleus varied obviously, there was apoptosis; Collagen fiber was increased nearby Smooth muscle cells, there was no other variation; mast cells had no variation.Conclusion (1) Distorted constitution of endometrial-myometrial junction is related to adenomyosis. (2) Mifepristone have an influence in endometrial-myometrial junction of adenomyosis.
