Objective:To explore the low serum bilirubin and LDLC / (HDLC + TBIL) ratio of whether coronary heart disease(CHD) risk factors .To study Serum bilirubin of the possible role of the mechanism of coronary heart disease. Methods: Potential risk factors were studied in 312 patients underwent coronary angiography, A totle of 312 patients were divided into two groups by their results of coronary angiography:1.CHD 2.Normal. Serum total bilirubin, high density lipoprotein cholesterol (HDLC),low density lipoprotein cholesterol (LDLC),LDLC/(HDLC+bilirubin)other traditional index were tested. their relation were investigated between two groups. The patients with coronary heart disease were divided into groups of one vessel disease, two vessel disease, three vessel disease. Comparison among the different groups were made in terms of serum bilirubin and LDLC/(HDLC+TBIL). analysis of correlation between LDLC/(HDLC+bilirubin) and other traditional factors were done. After the Analysis of receiver operating characteristics (ROC)curve for patients with coronary angiography and risk factors, Calculation of total serum bilirubin levels associated with coronary heart disease than the ratio of intensity was Calculated;Multivariate analysis using logistic regression analysis, regression model to be variable for coronary heart disease, serum total bilirubin, high-density lipoprotein cholesterol, smoking, sex, LDLC / (HDLC + TBIL) ratio as the independent variables. Results:â‘ CHD group serum total bilirubin, serum bilirubin, serum direct bilirubin were significantly lower than the control group (P <0.01). Coronary artery lesions with different array of serum total bilirubin, serum bilirubin, serum direct bilirubin did not achieve significant difference. The ROC curve analysis, the serum total bilirubin of coronary heart disease associated with the intensity ratio of 4.30, 95% CI (2.23-7.16). Logistic regression analysis, the serum total bilirubin entered the regression model.â‘¡Correlation analysis showed that, indirect bilirubin and serum triglyceride had a significant negative correlation (P <0.05), serum direct bilirubin and triglyceride significant positive correlation with high-density lipoprotein cholesterol was significantly negatively correlated (P <0.01).â‘¢CHD group of LDL-C / (HDL-C + TBIL) ratio were significantly higher than normal group (P <0.01). Coronary artery lesions with different array of LDL-C / (HDL-C + TBIL) ratio did not reach significant difference. LDLC / (HDLC + TBIL) ratio and body mass index was correlated (P <0.05). Logistic regression analysis, LDLC / (HDLC + TBIL) ratio also entered the regression model. Conclusions:â‘ Low serum bilirubin is an independent risk factor for coronary heart disease. Serum bilirubin on the risk of suffering from coronary heart disease have predictive value.â‘¡Serum bilirubin may be involved in lipid metabolism. Indirect bilirubin, direct bilirubin impact on lipid metabolism mechanism may be different.â‘¢Serum bilirubin and serum lipid composite index also serve as an independent risk factor for coronary heart disease, a comprehensive assessment of coronary heart disease risk has important clinical significance.
