Study On The Relationship Between Serum Ferritin, Transferrin And Type 2 Diabetes

Objective To investigate the serum concentration of serum ferritin(SF) and transferrin(TRF) levels in patients with type 2 diabetes(T2DM). To explore the relationship between iron store and T2DM.Methods Based on standards of WHO in 1999, 56 patients with T2DM were recruited as diabetic group, and 30 healthy subjects as control group. Meanwhile fasting true insulin(FINS), fasting blood glucose(FPG),blood lipid, blood pressure, height, weight, waistline and hip circumference were measured, and waist to hip ratio(WHR),body mass index(BMI) were also calculated. Glycosylated hemoglobin(HbA1C) was measured in diabetic group, additionally. The serum SF, TRF were measured with enzyme-linked immunoassay sorbent assay(ELISA). Insulin resistant index(HOMA-IR) andβfunction(HOMA-βcell) were calculated by HOMA formula. We explored if SF or TRF was correlated with HOMA-IR, triglyceride(TG), HbA1C, FPG, FINS and so on, respectively. In multiple stepwise regression analysis, the HOMA-IR was assumed as dependent variable, the other parameters as independent variables. We also analyzed the correlation of SF and TRF. All the data were input SPSS11.5 for statistical analysis.Result The levels of SF and TRF in diabetic group were significantly higher than those in control group, [(4.19±0.84) ng/mL vs( 4.65±0.88) ng/mL, P<0.05],[( 9.26±3.38) ug/mL vs (13.49±5.80) ug/mL, P<0.05],respectively;The SF was positively correlated with BMI(r=0.318,P=0.006),WHR(r=0.342,P=0.003),SBP(r=0.316,P=0.006),HbA1C(r=0.265,P=0.023),FINS(r=0.336,P=0.004),HOMA-IR(r=0.420,P<0.001).But it was negatively correlated with HOMAβ-cell(r=-0.342, P =0.003);The TRF was positively related to WHR(r=0.413,P<0.001),TG(r=0.420,P<0.001),FINS(r=0.318,P=0.006),HOMA-IR(r=0.414, P<0.001);there was no correlation between SF and TRF; In multiple stepwise regression analysis, only SF(r2=0.449, P<0.001),TRF(r2=0.223, P=0.005),WHR(r2=0.426,P<0.001), TG(r2=0.157, P=0.025)were positive predictor of HOMA-IR after adjusting for all potentially confounding variables.Conclusion The serum SF and TRF levels were increased significantly in patients with T2DM, Iron store is closely related with T2DM, including the clinical manifestation such as insulin resistance ,pancreasβ-cell function, lipid, body fat, blood pressure. Therefore, iron store can predicate type 2diabetes mellitus, and used as an important target of prevention and therapy for type 2diabetes.
