Objectives Aim of this study is to determine whether serum ferritin(SF)?Transferrin(TRF)levels relate to the different stages of HBV-related disease,and to investigate the clinical significance in HBV related Hepatocellular Carcinoma(HCC).Methods A cohort of 222 patients with HBV-related diseases patients were included in the study.They were divided into three groups: 74 cases of CHB group,64 cases of LC group and 84 cases of HCC.And we chosen 50 healthy individuals comprised the normal control group.The levels of various clinical indexes,including SF?TRF?AST?ALT et al were measured,and the results were analyzed by statistical methods.The ROC curve was used to compare the diagnostic value of different indexes in HCC.Results1.The median level of SF in HCC group was 284.200(484.400)ng/mL,which were significantly higher than that in LC group,CHB group and NC group: 167.350(348.205) ng/mL,129.650(235.592)ng/mL,101.950(126.708)ng/mL.There are differences among the four groups(T=29.241,P=0.000).The median level of SF in disease groups were higher than it in NC group,the differences between every group was statistical significant(P<0.003).2.The average level of TRF in HCC group,LC group,CHB group and NC group were as follows:(2.566±0.444)g/L,(2.724±6.189)g/L,(2.179±0.771)g/L,(2.9414±0.622)g/L.There are differences among the four groups(F=21.816,P=0.000).Further comparison showed that: the mean level of TRF in disease groups were higherthan it in NC group(P<0.003);The mean level of TRF in different disease groups: HCC group > CHB group(P=0.000);HCC group<LC group(P=0.128);LC group > CHB group(P=0.000).3.The patients with HCC was divided into AFP negative group and AFP positive group.The serum SF level were: 192.600(449.25)ng/m L,296.500(563.775)ng/mL,the difference was no statistical significance(Z=-1.259,P=0.208).The serum TRF levels in each group were: 2.255(1.103)g/L,2.210(0.830)g/L.There was no statistical difference(Z=-0.530,P =0.596).4.Throgh the Spearman correlation analysis,the levels of serum SF in HCC group were positively correlated with AST,ALT,TBIL,AFP(r=0.414,0.418,0.406,0.284,P<0.002),and were negatively correlated with AFP(r=0.252,p=0.044),but were no significant correlation with PTA(P=0.827).The levels of TRF were positively correlated with ALB,PTA(r=0.563,0.362,P<0.002),and were negatively correlated with AST,ALT, TBIL(r=-0.371,-0.311,-0.457,P<0.001);but were no significant correlation with AFP(P=0.299).5.The result of detecting the SF and TRF in HCC group was that the ROC curve of SF,TRF and AFP in diagnosis of HBV-Hepatocellular carcinoma was 0.699,0.659 and0.798 respectively,with diagnostic significance respectively(P=0.000).Conclusion The SF or TRF has a certain clinical value in the diagnosis of HBV related Hepatocellular carcinoma.Especially when the AFP was negative,joint detection of serum SF or TRF simultaneous may contribute to the diagnosis of HCC.
