Objective: To investigate the relationship between serum ferritin(SF)levels and insulin resistance and the indicators of metabolism in patients with IGT.Methods: From 1st May, 2008 to 1st May, 2010, in the out-patients of the endocrinology department, 182 patients with newly diagnosed IGT were recruited in the IGT group, 178 healthy subjects and 180 subjects with newly diagnosed T2DM in the control groups. The diagnosis was` made according to the WHO diagnosis standards of IGT and T2DM in 1999. After an overnight fast, all subjects underwent an oral glucose-tolerance test(OGTT). Plasma samples were collected before OGTT and 2 hours after. Measured parameters included levels of SF, fasting plasma glucose(FPG), 2-hour postprandial plasma glucose(2hPG), fasting insulin(FINS), blood Lipids (including total cholesterol, triglyceride, high density lipoprotein cholesterol, low density lipoprotein cholesterol) and blood pressure(BP), as well as height, weight, waistline, hip circumference, gender, age, menstrual history, alcohol abuse and other medical histories. SF was measured with chemiluminescence assay. Insulin resistant index(HOMA-IR)andβcell function(HOMA-β) were calculated by HOMA formula. We explored if SF was correlated with HOMA-IR, Body Mass Index (BMI), BP, blood Lipids and so on,respectively. In multiple stepwise regression analysis, The SF was assumed as dependent variable, the other parameters as independent variables. All the data were analyzed with SPSS11.5.Result : 1.The levels of SF in T2DM group were higher than those in IGT group, and the SF in IGT group were higher than those in NGT group. 2. In the analysis of the indicators of metabolism in IGT group, the SF was not significantly different in correlation with obesity, hypertension, elevated TC while it was significantly increased when associated with elevated TG, decreased HDL-C, or elevated LDL-C. 3,In IGT group, whether associated with obesity, hypertention, dyslipidemia or not, the insulin resistance index in the patients with elevated SF was higher than who without elevated SF.With the increase of SF, the degree and the incidence of insulin resistance increased.4,SF was positively correlated with TG,LDL-C,FPG,2hPG and HOMA-IR, negatively correlatde with HDL-C, and not correlated with BMI, SBP, DBP, HOMA-? and TC. In a stepwise multiple regression analysis ,HDL and HOMA-IR were the main factors affecting SF.Conclusion: The SF levels in IGT group were higher than those in NGT group, especially in the patients with elevated TG, decreased HDL-C, or elevated LDL-C. Increased iron storage was closely related to IR. HDL and HOMA-IR were the independent related factors affecting SF.
