| Tinea corporis is a kind of superficial fungal infection induced by pathogenetic fungi in human smooth skins(except skins of hands , feet ,hair, nail plates and vulvae). Tinea cruris , in fact, is a special type of tinea corporis on haunches and vulvae, and any circular and semi-circular dermal leisions on inner inguinal groove parts induced by pathogenetic fungi are called tinea cruris. Tinea corporis-cruris is a commom and multiple disorder, and its morbidity depends on gender and season differences. Male patients are more vulnerable than female ones, and people easily fall into this infection in summer and autumn . The sexual in differences on morbidity are associated with males'physiological features , hygienic habits and special jobs and seasonal indifferences are related to fungi which readily grow in warm and moist conditions. The pathogenetic fungi are Trichophyton rubrum, Epidermophyton floccosum , Trichophyton mentagrophytes, Microsporum cains and so on.Bifonazole is an anti-fungi broad-spectrum drug of imidazole produced by Germany Bayer Corporation in 1974,which inhibits biological synthesis of the fundamental components in the fungal membranes( ergosterin ) for bacteriostasis and sterilization . It acts more effectively than other imidazole derivatives on anti-fungi activities and spectrum, especially on dermatophytes, yeasts. Bifonazole has poor absorption performance but a high concentration in corneous layers, which is very beneficial for superficial anti-fungi effects.Triamcinolone Acetonide is a moderate externally applied agent of glucocorticoid , with a concentration of 0.025-0.1%, which acts as an anti-inflammator , anti- anaphylatic , immunosuppressive agent. Combination of anti-fungi drugs and glucocorticoid is used to treat superficial inflammatory mycosis, which relieves symptoms fast and improves effects. Bifonazole and Triamcinolone Acetionid - cream produced by LiaoNing DaSheng medicine limited company, including 1% Bifonazole and 1% Triamcinolone Acetonide, is a new cream product, without the same ones at home and abroad.Objective:To evaluate the safety and efficacy of Bifonazole and Triamcinolone Acetionid-cream on treatment of Tinea corporis-cruris.Method: Select 60 patients with fungal microscopy -positive tinea corporis-cruris from Nov. 2006 to Oct.2007, and conduct a randomized ,double-blind, parallel group trial with 1% Bifonazole cream (brand name FuQi) as a control. Recor the symptoms , signs, fungal testing outcomes and timelines of itch fading in diaries of the trial group and control group at the end of one week, two weeks after application and two weeks after with drawal. Meanwhile, document the adverse reactions. Ananlyse datd with statistical methods of t test andχ~2 test.Results:54 Males and 6 females are included ,with a ration of 9:1. 20 are patients with tinea corporis, and 40 with tinea cruris, with a ratio of 1:2.Among 60 cases, the number of patients with infection of Trichophyton rubrum is 41, with a proportion of 73.21%, Trichophyton mentagrophytes 4 ,and Microsporum cains 4,each with a proportion of 7.14%, Epidermophyton floc 1, Trichophyton tonsurans1, Candina albicans 1, Cladosporium 1, with a proportion of 1.7857%. After one week of application, mycological efficacy rate(fungal clearance rate ) is 76.67%, symptom-relieving rate is 60.48%, mend rate, 27.6%, acting rate 69%,and curing rate 3.4% in patients of the trial group. After two weeks of application mycological efficacy rate(fungal clearance rate ) is 100%,symptom-relieving rate is 86.84%,acting rate 58.6%, curing rate 41.4% in patients of the trial group. After two weeks of withdrawal,symptom-relieving rate is 99.26%,acting rate3.4%, curing rate 96.6%.There are no significant differences in mycological efficacy rate, symptom-relieving rate, improving rate, acting rate, curing rate between two groups after one week, two weeks of application and two weeks of withdrawal. However, there are significant differences between erythema - relieving rate and timelines of itch fading ,in other words, the trial product is better on erythema and itching relief. Only one patient suffered from slight adverse reaction , with a occurrence of 3.33%,but it faded quickly without special handling. No patients in the trial group suffer from biochemical abnormalities during this trial. Conclusion: Bifonazole and Triamcinolone Acetionid- cream is a kind of effective broad-spectrum antifungal medicine . Its characteristic of indulging in fat can improve the medicine resort in skin's stratum corneum for a long time and maintain high consistence exerting the medicine's effect. External use to cure Phlogistic cuticular fungal dermatosis , once a day. Period of treatment to cure tinea corporis-cruris is commonly two weeks. Using medicine for two weeks and stop using for two weeks. Using medicine for one week and stop using for one week. The curative effect has a prominent difference.Comparing Bifonazole and Triamcinolone Acetionid - cream with 1% Bifonazole cream(brand nameFuQi)a, there is no prominent difference on clinic curative effect, chief curative effect and mycology curative effect . Bifonazole and Triamcinolone Acetionid - cream compared with one 1% Bifonazole cream ( brand nameFuQi ) has a better anti- inflammation effect , can eliminating erythema and itch faster . The security of Bifonazole and Triamcinolone acetionid - cream is good. There is only one suffering from light ill reaction and the rate of ill reaction is 3.33%. Further more ,the ill reaction can disappear rapidly without special disposal. Comparing the situation , no sufferer abnormity of biochemical examination appears in the test group.
