| Background and objective : Rheumatoid Arthritis (RA) is a common systematic autoimmune disease .The main pathologic features include various joints invaded evolving synovial hyperplasia symbolized with chronic inflammation of joints and tissues . This Pathological changes can also been found in other tissues and apparatus. The course of disease is long .It outbreak repeat complex and levity. The treatment is difficulty. The prognosis is badness. RA is outbreak in all age. The common patient's age is 30~60 years. Especially 45 years around. The higher ages the heighten incidence of disease. Female is higher incidence than man .It shows that the incidence of RA is about 0.3~0.6% in China. 60% patient will be incomplete in 10 years. RA is one of main reason which made people lose labors force in China.The objective of therapy on RA is to control the process of disease. Alleviate pain and prevent bones from irreversible destruction .As well as to protect the function of joints and muscles and improve the quality of patients'lives. During these years western medicine has made great process in the research on RA treatment. But it completely different in therapies and efficient medicines dealing on bones invasion and joints destruct . It becomes urgent and exigency for us to explore and find out effective method and drug in treating RA.There is plenty of experience on RA in traditional Chinese medicine (TCM). The feature is mainly on the basic of planning treatment according to diagnosis and through multi-layer multi-loop multi-method and multi-target way with little side effect. During these years there is great process in the research of mechanism on Chinese medicine and many new compound or single Chinese medicine have been find in market recently. In the same time ignore on syndrome differentiation appears. So we all focus on the curative effect of syndrome differentiation in TCM.The research topic lies on the experience of professor Zhao Feng Da . The objective is to explore Bai Ying Decoction's curative effect and mechanism.Methods: Sixty RA cases which fit the ARA diagnosis standard (1987) are divided into two groups at random single blind. The patients of treatment group take orally BaiYing decoction 200ml bid except western medicine syndrome treatment and the patients of control group only accept western medicine syndrome treatment. After treatment 3 months . We observe the curative effect and main symptoms and signs .Such as arthralgia,arthrophyma,morning stiffness. We also test ESR,RF,CRP and T cell subgroup of patients such as CD~+4,CD~+8.Results: The clinical total effective rate of treatment group is (93.33%) which significant difference(P<0.05)surpasses the effective rate is ( 73.33% ) of control group. After treatment the main symptoms and signs are much better than before. The sensitive parameters such as ESR,RF and CRP have been lowered to some extent or got back to the standard level. BaiYing decoction has function of regulating cell mediated immunity lower the level of CD+4 and upper the level of CD+8.. regulate T cell subgroup and keep the balance of CD~+4/CD~+8.Conclusion: BaiYing decoction is an effective prescription that treats the patients of RA. The mechanism is probably immune regulation .so we think that the prescription is worth researching further for the purpose of developing a new drug to treat RA.
