Objectives:(1) To analyse the effects of yunnan baiyao toothpaste on crevicular prostaglandin E2 (PGE2). (2) To evaluate the effects of yunnan baiyao toothpaste on the reduction of gingivitis, gingival bleeding and plaque.Methods:(1) Part 1: 22 adults with moderate-serve gingivitis of molars were recruited. Two molars(not including 3rd molars) with gingival inflammation were selected in each subjects and gingival crevicular fluid (GCF) samples were taken from 3 sites in each tooth. Gingival index (GI) and sulus bleeding index (SBI) were recorded. Subjects were randomly divided into either the test group (yunnan baiyao toothpaste) or the control group (placebo toothpaste). Both groups used their assigned toothpaste twice a day for 3 min toothbrushing. GCF collecting and clinical examination(GI and SBI scoring) were done at days 60. A euzymelinked immunosorbent assay kit was used to determine GCF- PGE2 .(2)Part 2: Ninety-eight healthy adult volunteers from chengdu city were accepted as subjects for this six-month, double-blind, parallel-group clinical trial. Sulus bleeding index (SBI) and plaque index (PLI) were recorded at baseline. Following a dental prophylaxis and instruction in brushing technique, the participants were randomly assigned to either the test group (yunnan baiyao toothpaste) or the control group (placebo toothpaste).The indexes (SBI, PLI) were reexamined at month 3 and 6.Results:(1)Part 1: Statistical analysis indicated no significant difference in laboratory indexes and clinical indexes between the two groups at baseline. Compared with the control group, there was a significant decrease in GCF volume from 1.28ul to 0.96ul and so was in the total amount of PGE2 in GCF from 34.22pg to 23.82pg, after using yunnan baiyao toothpaste for toothbrushing for 2 months. A slightly decrease in GCF-PGE2 concentrations was observed but was not statistically significant when compared with the control group. There was constantly significant correlation between GI and GCF volume ,so was between GI and PGE2 total amount and between SBI and PGE2 total amount. Significant correlation between SBI and GCF concentrations was found only at baseline. (2)Part2: Statistical analysis indicated no significant difference between the two groups at baseline. At the 3rd month, statistically significant decrease in SBI values were found for both of the two groups. The SBI values and PLI values in the test group were lower than those in the control group by 16.0% and 4.94% respectively. The differences in SBI values between the two groups were statistically significant but no significant statistical differences were observed in PLI values. At the 6th month, statistically significant decrease in SBI values and PLI values were found for both of the two groups. The SBI values and PLI values in the test group were lower than those in the control group by 10.11% and 2.75% respectively. However, there was no significant difference in these indexes between the two groups.Conclusions:(1)Using yunnan baiyao toothpaste for toothbrushing resulted in a significant decrease in GCF volume and GCF-PGE2 total amount during this 2-month trial period. (2)Using yunnan baiyao toothpaste for toothbrushing resulted in a significant reduction in SBI values at the end of the 3-month period. But there was no difference between the toothpastes in the reduction of PLI and SBI at the end of the 6-month period.
