Objective To study the hemodynamic changes of regions surrounding hematoma in patients with hypertensive intracerebral hemorrhage ( HICH) by 16-detector spiral CT perfusion imaging ( CTPI ) .Method Routine CT and PCTI imaging were performed with 16-detector spiral techniques in 28 patients with HICH. 2 weeks after first check, the patients were reexamined and cerebral blood flow, cerebral blood volume , mean transit time of tissue surrounding hematoma and opposite side corresponding regions were measured and quantitatively analysed for comparing.Results In 28 patients , CBF and CBV in regions surrounding hematoma were lower than normal regions,and MTT in regions surrounding hematoma were delayed than normal regions in acute stage.CBF((19.11±13.75)ml/min·100 g )surrounding hematoma were significantly lower than normal sides (50.26±43.86)ml/min·100g (P<0.05)), CBV ((1.16±0.49)ml/g) in regions surrouding hematoma were less than normal sides ((2.28±1.26)ml/g(P<0.05)),and MTT ((6.76±3.51)s) in regions surrounding hematoma were significantly delayed than normal sides((4.98±2.96)s(P<0.05) ) in subacute stage. CBF were not significant different between acute stage and subacute stage,but CBV(1.40±0.67)ml/g and MTT(8.19±2.67)s were significant different between acute stage and subacute stage( P<0.05).Conclusion CBF and CPP of the regions surrouding hematoma in patients with HICH are lower than the normal regions, and valuable information about clinic therapy for patients with HICH can be provided through quantitatively analysed hemodynamic changes surrounding hematoma by PCTI .
