Objective To evaluate the safety and feasibility of laparoscopic hepatic left lateral lobectomy.Methods Clinical data of 15 patients from December 2004 to January 2007 were analyzed, including10 cases of hepatocellular carcinoma, 3 cases of hepatic hemangioma,and 2 cases of metastatic liver carcinoma from colorectal carcinoma,Tumors were all located at the left lateral lobe.under laparoscopy ,the ultrasonic dissection,electrocautery,or linercatter ligation were utilized to perform regular hepatic lobectomy. Procedure included introduction of trocar; isolation of the liver from the ligment;dissection of the hepatic parenchyma by the ultrasonic dissection,electrocautery,or Endo-GIA and removal of the samplesResults laparoscopic hepatectomy was uneventfully accomplished in all the 15 patient ,without conversions to open suegery . The operation time was 65~120min (mean ,95 min),and the intraoperative blood loss was 80~300ml (mean,140ml). The tumors were entirely resected with a resection margin beyond 1cm .The pathological fingings showed negative surgical margin.No hemorrhage or bile leakage were observed after operation.The abdominal drainage tube was indwelled for 3~4 d. The Postoperative hospital stay was 3~6 days. Follow-up checkups for 1~20months (mean,12.5 months). No tumor or incision implantation was found.All patients were survived.conclusions Laparoscopic regular hepatic left lateral lobectomy can be carried out safely and effectively for the selected patients.
