Function And Mechanism Discussion Of Leptin On The Course Of Acute Pancreatitis

Objective Through studying the change of acute pancreatitic(AP)patients' leptin level of blood serum,discuss the relations between leptin and interleukin-1β(IL-1β),tumor necrosis factor(TNF),interleukin-4(IL-4),body mass index(BMI),body fat content(%Fat)and so on. Furthermore,propose the function of leptin in the AP and its possible action mechanism.Methods Collected 30 acute pancreatitic patients who are diagnosed on the acute pancreatitic diagnostic criteria which is drawed up by the pancreatitic group of Chinese Medical Associaition' s surgical institute and is defined by the patient history,symptoms,physical signs,laboratory examinations and imageology.In these patients,the ratio between male and female is 14:16. They are from 20 years old to 73 years old and the mean age is 42.2 years old.They were drawn blood when they were in hospital after morbility and were therapied two months later.Thus,they were divided two groups:acute pancreatitis group and the recuperative pancreatitic group.The control group which has 30 example were healthy crowd who were matched in ages and body weights with the 30 patients and they all have not the acute or chronic liver disease,hypertensive disease,coronary artery disease,diabetes,obesity and so on.Determine the leptin level of blood serum in the AP group with 30 examples,the recuperative AP group with 24 examples,the control group within 30 examples by means of radio-immunity.Simultaneously determines the IL-1β,TNF,the IL-4 level of blood serum and figures out its BMI,the%Fat.Deal with the datum by SPSS11.5 statistic software and demonstrate the datum with x±s.Group comparison uses the LSD method.The relationship between leptin and each index are stated by Pearson correlation analysis.Results The leptin level of blood serum in the AP group is higher than the recuperative AP group(P＜0.001).The difference between the recuperative AP group and the control group does not have statistic significance(P=0.718).Also,leptin have the positive correlation with the BMI, the%Fat,IL-4 and have the negative correlation with IL-1β,TNF.Conclusion The result of this experiment has indicated that plasma leptin levels in the acute pancreatitic patients is higher than that of the recuperative pancreatitic patients and the normal control(P＜0.001).Therefore,the leptin levels of blood serum have the close relation with the genesis,development and turnover of the acute pancreatitis.Furthermore,the elevating level have the close relation with the changes of TNF,IL-1β,IL-4,BMI,and%Fat.In this experiment,the correlation analysis between leptin and the nutrition parameter:BMI,body fat content and blood fat demonstrates that leptin has the significance role in maintaining the balance both in body weight and in energy metabolism.Also,leptin have the positive correlation with IL-4 which inhibit inflammation and have the negative correlation with the lever of proinflammatory cytokine:IL-1β,TNF.Accordingly,leptin reverse the progression of AP, through increasing the lever of IL-4 and decreacing the lever of IL-1βand TNF-αin the blood serum.
