A Study On The Association Between The Interleukin-1β,Interleukin-6 Gene Polymorphisms And The Condition Of Patients With Acute Pancreatitis

Objective To investigate the relation of IL-1β-511C/T,IL-6 -174G/C,-634C/G gene polymorphisms in the promoter region and susceptibility to acute pancreatitis(AP) and the condition of patients with AP .Methods The polymorphisms of interleukin-1βgene -511C/T and interleukin-6 gene -174G/C,-634C/G were analyzed by polymerase chain reaction– restriction fragment length polymorphism(PCR—RFLP) and DNA sequence analysis methods in patients(n=74) with AP(MAP 36,SAP 38,not SIRS33,SIRS41) and healthy control(n=78) .The distribution frequencies of the genotype and allele were compared among different groups. The plasma concentrations of IL-1βand IL -6 were measured by ELISA, and the plasma concentrations of IL-1βand IL -6 were compared among different groups.The patients with SAP were divided into groups on the basis of differet genotypes,and th relative clinical date(the scores of Balthazar CT,APACHEⅡand MODS) were compared among different groups and the score of Balthazar CT and APACHEⅡwere compared between SAPⅠand SAPⅡ.The statistics software SPSS13.0 was used to finish't'test and'χ2'test.Results (1) IL-1βand IL-6-634 genetype frequency of AP patients was not significantly different from that of healthy controls(P>0.05).In patients with AP, IL-1β-511T/T genetype and T allele frequency of SAP was significantly higher than that of MAP ,so were in AP patients with SIRS and the patients without SIRS.IL-6-634C/G genetype and G allele frequency of AP patients with SIRS was significantly higher than that of the patients without SIRS. The mutation frequency of IL-6-174(C/G) is small, and the genotype frequency and allele frequency were not significant differences among different groups. (2) The plasma levels of IL-1βand IL-6 in patients with AP were significantly higher than that in the healthy controls(P<0.05),There was no significant difference in plasma IL-1βlevels between C/C group and C/T+T/T group(P>0.05), and the plasmalevels of IL-6 in patients with IL-6 -634 C/G were significantly higher than C/C group(P<0.05). (3) In patients with SAP ,the scores of Balthazar CT,APACHEⅡand MODS in IL-6 -634 C/G group were significantly higher than that in IL-6 -634 C/C group(P<0.05),and the score of APACHEⅡin IL-1β-511C/T+T/T group was significantly higher than that in IL-1β-511C/C group(P<0.05). In patients with IL-6 -634 C/G genotype, There was no significant difference in the score of APACHEⅡbetween the first day and the forth to seventh day in hospital(P>0.05 ) . There was significantly reduction in other groups. The scores of Balthazar CT and APACHEⅡin SAPⅡwere significantly higher than that in SAPⅠ.Conclusions The plasma concentrations of IL-1β,IL-6 were important characteristics to acute pancreatitis(AP).IL-1β(-511C/T) and IL-6(-634C/G) gene polymorphisms are not related to the susceptibility to acute pancreatitis , but the genotype and allele frequency are associated with severity of AP. The T and G allele may worsen AP. IL-6(-174G/C) gene polymorphisms are not associated with susceptibility to acute pancreatitis and severity of AP. IL-6(-634C/G) gene polymorphisms may influence plasma levels of IL-6 , and IL-1β(-511C/T) gene polymorphisms may be not associated with plasma levels of IL-1β.
