Object: Intrcerebral Hemorrhage(ICH) is a sereve style of cerebrovascular diseases, belonging to the category of stroke in Traditional Chinese Medicine(TCM). Since the view of Qixu Xueyu proposed by a famous doctor of TCM, named Wang Qingren, who initiated the prescription of Buyang Huanwu Decoction, Yiqi Huoxue therapy has been widely used in clinic. Modem medical researches have proved Yiqi Huoxue therapy can improve the circulation in the brain hemorrhage zone and accelerate the absorption and healing of hematoma. However, the capillary vessel system in the focus infected was damaged in the course of hemorrhage because of mechanical destruction and the extensive blood endothelial cells damage through inflammation. Only by the restruction of capillary vessel system origining from the angiogenesis and vasculogenesis, the reperfusion in the hemorrhage bulk would be guaranteed. In the course of angiogenesis, vascular growth factor mainly include vascular endothelial growth factor, VEGF family, angiopoietin, Ang family and Pleiotropic Angiogenic Factors. Among them, Ang-1 is a growth factor that specifically acts on the blood endothelial, Tie-2 is a receptor for Ang-1; Ang-1 is combined with Tie-2 receptor by secretion, triggering the interaction between the vascular endothelial cell and blood vessel cell, making the blood vessel integrality, maintaining steady and ripe state. Therefore, we suppose that the mechanism of Yiqi Huoxue therapy used in ICH is a course of adjusting expression of Ang-1 and its receptor Tie-2, promoting'restructing of capillary vessel system in injured region by angiogenesis, striving the reperfusion early, accelerating the absorption of hematoma and promoting the recovery of neurofunction. The objective of this experiment is to investigate the effect of Yiqi Huoxue therapy on expression of Ang-1 and its receptor Tie-2 in the blood endothlial cells, to study the mechanism of YQHX therapy on reconstitution of capillary vessel system in the hematoma area after ICH initially.Methods: 288 rats were randomly divided into six groups including normal control group, sham control group, model control group,sham control group,Yiqi therapy group,HuoXue therapy group and Yiqi Huoxue therapy group. By injecting collagenase typeⅦstereotaxically into fight globus, intracerebral hemorrhage(ICH) model was made. Yiqi therapy group,HuoXue therapy group and Yiqi Huoxue therapy group were respectively administered at YiQi decoction(8.10g/kg.d),HuoXue decoction(7.30g/kg.d) and Buyang Huanwu decoction(15.40g/kg.d), At 1d, 2d, 4d, 7d, 14d, 21d and 28d, rats were randomly chosen to be perfused transcardically, the changes of brain were observed by hematoxylin eosine staining (HE) and Immunohistochemistry was used to examine the rule on the expression of Ang-1 and its receptor Tie-2 in vascular endothelial cell nearby the haematoma bulk, and observing the effect of Yiqi Huoxue therapy.Results:1. morphological changes in the rats's brain is observed by HE stainingNo positive pathology change and hematoma appeared in brains of normal or sham group during the experiment, model group at 4d, obvious edema, necrosis, inflammation cells soakage such as lymphocyte, a small amount of neutral cells appeared, hemosiderin to deposit can be seen in the outer fringe of hematoma, which is more slighter in other three groups and have a small amount of irregular blood capillaries appeared around hematoma, Yiqi Huoxue group is the most obvious; model group at 14d, irregular new born blood vessel around model hematoma outgrow, the erythrocyte basically dissolved, the scattered colloid cell appear at hematoma edge, by 21d, hematoma has basically disappeared. And for Yiqi Huoxue group, disappearing time of hematoma advance to14 day, hematoma district can see obvious blood vessel section, positive expression of section of blood vessel at hematoma district are the most obvious in Yiqi Huoxue group than other three groups.2. the results of ImmunohistochemistryThe expression of Ang-lin blood vessel cells of basal ganglia nearby hematoma: accidental and fragmentary expression in normal or sham group at different time, no obvious difference in healthy side and unhealthy side. After ICH, for mould group the expression amounted slowly at 1d and elevated notably at 4d. However, on reaching the climax at 21th day, it attenuated slowly at 28d. And in Yiqi therapy group, at 4th day, the expression significantly increased. In Huoxue group, at 2d, the exression was strikingly stronger than the former(p<0.01). At 14d, it reached the pointand was higher than that in model group and YiQi therapy group. In Yiqi Huoxue therapy group, at 1d, the expression increased the most notably immediately that in other three groups(p<0.01). At 14th day, it reached the point. And then, the expression became to be weaker slowly. But at 21th day, the lever was still higher than it in other groups at same time(p<0.01). With the time, there is a tendency of the expression of angiopoietin-1 and receptor Tie-2 in microvascular networks after ICH: amounding gradually in the beginning and descending gradually in the later.2. Yiqi Huoxue therapy has a positive effect on the promotion to the expression of angiopoietin-1 and receptor Tie-2, deference in the descending, maintaining a suitable lever, which would do some helps to accelerating the microvasculogenesis in enhaematoma bulk zone, recovering reperfusion to focal zone and the function of the rats'nerve.
