Acute pancreatitis is one of the most common acute abdominal diseases in China and its morbidity seems to be rising year after year. The researches on its pathogenetic mechanism and therapies have now become important research subjects at home and abroad. Due to the appearance of advanced research means and methods and the hard work of researchers all over the word people have known more about the pathological process on AP. Recently researchers universally attach importance to the efforts of changes in pancreatic cells or intracellular Ca2+overload more accuracily , in the pathophysiological process of AP. In this research we investigate the effort of Ca2+overload in producing and developing AP by inducing the acute pancreatitis(AP) model of rats . We also get a exact curative effect by treating SAP with Tetramethylphylpyrazine (TMP) and explore its possible mechanism.Objective: TO investigate the effect of Ca2+ overload in the producing and developing of acute pancreatitis and to explore the precise effect of TMP in the treatment of AP model and its possible mechanism.Methods: The AP model of rats is induced by retrograded injection of 5% sodium taurocholate into the pancreatic duct. The rats were divided into three groups dealing with different measures: sham-operated group, severe acute pancreatitis (SAP) group and severe acute pancreatitis treated with TMP group. Adopted by cardiac puncture for blood sampling in the rats, the plasma levels of AMY, SOD, MDA and intracellular Ca2+were measured and the pathologic changes of pancreas were observed. Observed the above indexes and the level changes of Ca2+ in pancreatic acinar cell, free radical in these groups. Then to explore the effect of Ca2+ overload in the producing and developing of acute pancreatitis , the precise effect of Tetramethylphylpyrazine (TMP) in the treatment of rat SAP model and its possible mechanism.Results: The level of AMY serum amylase in sham-operated group, showed no significant changes. The levels of AMY serum amylase in AP group and TMP group increased significantly after 2 hours, peaked at 6 hours., and then gradually declined. Compared with the sham-operated group, the levels of AMY serum amylase in AP group and TMP group at all the time points were significantly higher than that of group A (P<0.05). When compared with the control group, the level of AMY serum amylase in TMP group at the same time point was lower significantly(p<0.05). After shaping the sham-operated model, the level of intracellular Ca2+concentration in pancreatic acinar cell had little changes. On the contrary, the.SAP and TMP groups were higer than that of sham-operated group all the time (p<0. 05). After shaping SAP model, the level of intracellular Ca2+concentration in SAP group cell was significantly higher than the two other groups, it was 1. 9 times that of ham-operated group and 1.4 times that of TMP group after 6 hours, 2.8 times that of ham-operated group and 1.5 times that of TMP group after 12 hours (p<0.05). After shaping sham-operated model, the levels of MDA and SOD in sham-operated group had little changes all the time , but the two other groups did not like this, the level of MDA in SAP group and TMP group increased significantly , the level of SOD in SAP group and TMP group decreased significantly (p<0. 05). When compared with the control group of TMP group at the same time point, the level of MAD increased but that of SOD decreased slightly, the two groups had an apparently difference(p<0. 05). It showed that the pathologic changes of pancreas in the TMP group had occurred late and the performance was relatively light compared with SAP group by observing the pancreas. We evaluated the score of the pancreatic tissues using optical microscope . The score of SAP group and TMP group were significantly higher than that of sham-operated group (p<0.01), but the score of SAP group were significantly higher than that of sham-operated group (p<0.01). Tetramethylphylpyrazine (TMP)suggested that played a protective role in the pancreas of rats.Conclusion: Ca2+overload may be involved in the development of acute pancreatitis. Tetramethylphylpyrazine (TMP)could significantly reduce pancreatic intracellular Ca2+ concentration and the level of free radical . Furthmore, it played a protective role in the pancreas and many organs of rats. These findings suggested that Tetramethylphylpyrazine (TMP) correlated with a better prognosis.
