Objective To investigate and find out the structure and distribution of the medical staff of Changde, as well as the wage and subsidies of the medical staff, and analysis the problems that exist in the current wage system, then advance same policy recommendations about the health staff structure and pay-related measures.Methods This study is a research about health staff in addendum in all medical institutions under posts in Changde. Data mainly came from the basic information survey which include: gender, age, seniority, qualifications, Job Titles, education, profession, monthly basic wages and subsidies information. And use SPSS11.5 to analysis those information.Results By the end of 2004, the urban population of Changde City is 1.37 million, per 1,000 population have 3.2 beds, 4.7 health staff, 2.0 the practice (Assistant) physician, 4 registered nurse; In third and secondclass medical institutions, the ratio of practice and nurse is 1:1.08; The ratio of advanced: junior: middle: primary and below in the third-grade hospitals is 1:4:13:10:2, the ratio in second-class hospitals is1:6:52:68:34, in first-class the ratio is 0:1:5:61:54; In clinics, the ratio of health staff with college education or higher is only 7.8%. Significant differ- rence exists in wages and income because of difference in gender, age, seniority, qualifications and titles of the medical staff and medical institutions. Different amount and type of subsidies exists in various levels of Medical institutions, the lowest subsidies is only 16 yuan, the most is 363 yuan-21 times larger than the former.Conclusion The allocation of health resources in Changde City is inconsequence, for example, the relative shortage of nurses, the number of doctor Job Title is small in both ends and large in the middle of the structure, the overall academic levels of primary health care units is relative low; Meanwhile, in medical institutions of different levels, income levels varies, even in the same levels, income difference exists in different units; The higher the level is, the higher the wage is, and in same levels, wage in general hospital is higher than specialist hospitals; the average wage and supervise of medical professionals and clinical practitioners is higher than public health professionals and health workers; medical institutions in different levels implement different type and amount of supervise.Recommendations Updating the concept of wages, strengthen salaries awareness; Improve the management of payroll, create the total wage bill evaluation mechanism; Improve the personnel system reform, and promote the implementation of the pay reform of the distribution system; Adhere to the principle of t fair and stable health staff to ensure the effectiveness of incentive pay and create various distribution methods; Promote knowledge-oriented, performance-oriented, ability-oriented and market-oriented incentive mechanism; implement various social security system, improve staff benefits.
