Objective: This study aimed to analyses the expression of basic fibroblast growth factor (bFGF) in human vocal fold polyp. We studied the pathogenesis of vocal fold polyp further in order to afford evidential to elucidate vocal cord polyp and find new means to treat it for the future.Methods: The expression of bFGF was detected in the examples of 37 vocal fold polyps (allocate into the two groups according to course of disease≥6 month 18 case and<6 month 19 case;and allocate into the two groups according to sex of male 24 case and female 14 case) and 18 healthy vocal folds with immunohistochemical staining (SP method). Quantitative image analysis was performed with computer assisted pathological image analysis system.Results: The expression of bFGF was observed in cytoplasm, no staining in cytoblast. The expression of bFGF was found in normal vocal cords and vocal cords polyps the different positive in degree responds. bFGF positive staining mainly find in epithelial cells and endothelial cells. In the normal vocal fold, however, and we observed accidentally weak positive expression, and of which the positive expressive rate was 16.67%, the positive cell number was 8.81±4.08, In the two groups of vocal fold polyps. The positive expression was stronger in epithelial cells , endothelial cells , inflammatory cells(primary cells are plasma cells and macrophage ) in the proper lamina of the mucosa, endothelium cells and fibroblast under mucosa. In the course of vocal fold polyps<6 month , the positive expressive rate was 63.16%. positive cell number was29.45±19.01.In the course of vocal fold polyps≥6 month , the positive expressive rate was 66.67%. positive cell number was 36.19±23.23 . The positive expressive rate in the male group was 66.67%, the female group was 61.54%, and the difference was not statistically significant (p> 0.05) . The percent of positive expression of bFGF and the positive cells counts in two groups of vocal fold polyps (course of diseases≥6 month and course of diseases<6 month) were significantly more than in normal vocal fold(p<0.05) . The quantitative analysis on immunohistochemical image of results: in the normal vocal fold, the mean integral optical density (MiOD) value of bFGF was 15664.36±7589.42, and the value of average positive expression area was 2.20±1.06μm~2.The MiOD values of bFGF were 44564.38±26591.60 (≥6 month of groups) and 33037.13±16986.11 (<6 month of groups), the values of average positive expression area were 6.79 + 4.29μm~2(≥6 month of groups)and 4.90±2.47μm~2(<6 month of groups). There were statistically significant between the MiOD values and the values of average positive expression area in vocal fold polyp tissues and that in the normal vocal fold (p<0.05) . This expression of bFGF in the two groups of vocal fold polyps tended to be increased greatly compared with that in normal vocal folds. But the percent of positive , the number of bFGF cells, the MiOD values of bFGF and the value of average positive expression area were insignificant between the two groups of different course of vocal fold polyps (p >0.05) .Conclusion: The bFGF may contribute to some of the pathologic changes observed in vocal cord polyps. The bFGF may induce epithelial cells proliferation and the neovascularisation of proper lamine and the extracellular matrix accumulation. The bFGF may play a key role in the neovascularisation and formation of heavy edema of vocal cord polyps. The bFGF may be involved commonly in the formation and development of vocal cord polyps . We may conclude that cytokines play a great role in the etiology of vocal cord polyps. If we can control the produce of cytokines (bFGF) in the vocal cord polyps, we may find a new way to treat it. But, the bFGF may have nothing to do with course of disease participating in the occurrence of the vocal cord polyps, nothing to do with sex.
