Background and ObjectNasal polyp is a disease of chronic nasal mucous membrane-nasal sinuses mucousmembrane inflammation, as a very common disease of human respiratory tract, itis a very severe threaten to human health and brings down patient' s quality oflife. A lot of articles reported the pathogenesis of chronic nasal mucous membrane-nasal sinuses mucous membrane inflammation is very complicated and unclear, soexact etiology and pathogenesis of nasal polyp are still under debate, ln the past10 years, the role of cytokines in the pathogenesis of nasal polyp is very arrestive.Recently, A lot of articles reported the pathogenesis of nasal polyp was anoverbalancing course of cell proliferation and apoptosis, and some researchersreported the abnormal apoptosis of eosinophil along with the abnormal expressionof much cytokines in nasal polyp. Therefore, regulated the chronic inflammation ofnasal polyp by modulateing eosinophil apoptosis and accommodateing the Cytokinein the micro-environment of nasal cavity has become the new field to discover nasalpolyp' s pathogenesis.IL-23, discovered by Oppmann B in 2000, is a new member of Cytokines familyof IL-12. Lots of researchs indicated it had a similar bioactivity with IL-12, bothof them play an important role in early inflammation. After IL-23 molecule linkingits receptors, it leads to the signal transduction way of JAK/STAT3 activation.STAT3 can enhance Bcl-2 and Bcl-xl expression excessive, lt was repored that Bcl-2and Bcl-xl were expressed strongly in nasal polyp, that indicated Bcl-2 and Bcl-xlplayed an important role in the pathogenesis of nasal polyp.So, we assume IL-23 plays an important role in the pathogenesis of nasal polyp.As yet, we didn't found any report about this. So we detect the IL-23 proteinand its Induce-factor IFN-γprotein by ELISA in nasal polyp, and also detect the expression of Stat3 by Immunohistochemistry, Analyze the effect route ofIL-23, probe into the function and approach of IL-23 in the pathogenesis of nasalpolyp, especially enhance our knowledge about the effect of cell apoptosis andcytokine in the pathogenesis of nasal polyp, therefore, we can provide experimentaland academic base for farther research of the effect of IL-23 in the pathogenesisof nasal polyp.MethodsAll our cases that measure up the standard were from the ENT department inFt affiliated hospital of Shantou university medical college, 20 samples of nasalpolyps were obtained from patients undergoing nasal polypectomy, and 10 samplesof inferior nasal concha from patients undergoing nasal septal construction ascontrol. Every samples divided into two parts, and do these laboratory testingbelow after proper processing.1. Collect clear supernatant after homogenate, and then detect the contentof IL-23 and IFN-γby ELISA.2. detect the expression of Stat3 by Immunohistochemistry.Results1. The expression of IL-23 protein in nasal polyp is lower than the expressionin inferior nasal concha, and the difference was significant (P<0.05).2. The difference expression of IFN-γbetween nasal polyp and inferior nasalconcha was not significant(P>0.05).3. The expression of Stat3 in nasal polyp is higher than the expression ininferior nasal concha, and the difference was significant (P<0.05).And most ofthe Stat3 express in cytoplasm, some in cell nucleus.Conclusions 1. IL-23, as a member of Cytokines of Th1, will decrease on the condition ofsome pathogenies. And it will change the proportion of the Cytokines of Th1/Th2in the micro-environment of nasal cavity So we presume IL-23 maybe plays animportant role in the pathogenesis of nasal polyp.2. IL-23 possibly promotes Bcl-2 and Bcl-xl excessive expression byregulateing the signal conduction way of JAK/STAT3. And it will cause theaccumulation of eosinophil cells. This mechanism maybe plays an important rolein the patbogenesis of nasal polyp.3. IL-23 maybe promotes the apoptosis of eosinophil cells. When IL-23 decreases,the apoptosis of eosinophil cells become slowly. And it will cause the accumulationof eosinophil cells. This mechanism maybe plays an important role in thepathogenesis of nasal polyp.4. The expression of IFN-γin the nasal polyp is different between the orientaland the westerner, that is worth of further research.
