Objective: To investigate the characteristics of inflammatory cell infiltration in nasal polyps(NP)organization,exploring the clinical characteristics of NP that neutrophil infiltration is predominant cells,analysing the expression of interleukin-17(IL-17)and interleukin-8(IL-8)in these tissues,in order to provide the clinical classification and therapeutic strategy for nasal polyps.Methods: Collecting March to October 2016 years in our department of the nasal endoscopic surgery these NP specimens According to pathological characteristics,Referring to the manner used by Wenzel SE et al[1],NP was classified as the inflammatory cell dominant role when the percentage of the inflammatory cell > the twice SD ± the mean of the normal control,distinguishing neutrophils nasal polyp(NEUNP)or eosinophils dominant nasal polyps(EOSNP),collecting 40 patients respectively,choosing the control group from the normal inferior turbinate mucosa in 20 patients with nasal septum deviation diorthosis.Comparing NEUNP with EOSNP,the regular examination the peripheral blood neutrophils and eosinophils percentage.observing the sinus CT,visual analog scale(VAS)and nasal endoscopy used by symptom scores to evaluate clinical data before operation,analysing the clinical characteristics,examining IL-17 and IL-8 in nasal polyps and the normal inferior turbinate mucosa by immunohistochemistry.Results:The study that these neutrophils > 4%(1.25%+2×1.37%=3.96)define as NEUNP,these eosinophils>8%(4.4%+2×1.7%=7.9%)define as EOSNP.Compared with EOSNP,NEUNP is no difference in gender,age and neutrophil percentage(p>05.0).The nasal stick purulent secretion mean score in NEUNP is higher than EOSNP(6.35±1.20 VS 5.69±1.31,p<05.0),VAS and nasal polyps lesion score between two groups respectively show no obvious difference(p>05.0).Sinus CT overrall score in NEUNP patients are lower than EOSNP(9.59±3.35 VS 14.43±3.81,p<05.0),maxillary sinus score between the two groups present a significant difference(3.14±1.21 VS 2.05±1.02,p<05.0),NEUNP patients has a low lesion,but maxillary sinus lesion becomes more severe.IL-17、IL-8 in NEUNP are respective higher than in EOSNP and the normal mucosa(p<05.0)Conclusion: NEUNP patients’ nasal secretion is stick purulent,which subjective symptoms score are apparently higher than EOSNP patients.The former patients’ maxillary sinus lesions are also apparently higher than the latter patients.IL-17 and IL-8 in NEUNP tissues are respectively higher than EOSNP and the normal inferior turbinate mucosa tissues,implying IL-17 and IL-8 chemotaxis neutrophils into nasal polyps,which play an important role in the pathogenesis of NP.Therefore,theere are obvious characteristics in NEUNP,which provide new understanding for the mechanism of NP.
