Study On The Traditional Chinese Medicine Types Of Syndrome Of Chronic Hepatitis B

Objective:There is a long history on the study of the hepatitis in the traditional Chinese medicine. As a fact; China has the most population of hepatitis B (HBV). Traditional Chinese Medicine play an important role in treating HBV in China, and the differentiating syndrome is the key to this problem. The clinical behavior of this disease is variable, and the pathological changes happens in liver. Deficiency of healthy energy and concentration of evil is the major causes of liver disease, the instinct of this disease is deficient but the appearance is overflow.Stagnation of liver-energy and dysfunction of the spleen in transporting and distributing nutrients and water happens in the all stages of this disease. In recent years, the study on the TCM syndrome of chronic HBV had made some progress, but still cant find the breakthrough point. In order to find out the relationship between the serologic examination and TCM types of the syndrome, the most common syndrome type of chronic HBV patients, we hold this subject. Finally, we will find out the instinct of syndromes and to seek for the rule and relationship of different syndromes and to develop the outliner or the clinical treatment. To help the patients with this disease is our eternal aim.Methods:Take the method of observing the cases and laboratory examinations. The 120 cases were took from the clinical and in-patient department in January 2006 to February 2007. They were diagnosed as chronic hepatitis B. Write down the detail information including the medical history, symptoms, syndrome type, and the lab examination as ALT, AST, GLB, TBIL, TBA, ALP, HBV-DNA, and PCⅢ, CL-IV, LN, HA, etc. And then, collect all of these information and made a database, get the result by SPSS11.5. software, manage the descriptive statistics, use the rank sum test to manage ordinal data, use analysis of variance for quantitative data, and rank sum test if it is not homogeneity of variance, 2-tail, a=0.05.Results:In 120 cases of this study, 87 cases are the stagnation of liver-energy and spleen deficiency type, it is 72.5% of all; and the followings are: the retention of evil wetness-heat type, accumulated blood stasis type, the liver and the kidney yin-deficiency type, the spleen and the kidney yang-deficiency type. The data are respective to 24, 3, 3. The stagnation of liver-energy and spleen deficiency type is the most common in most cases.The sequence of HBV patients of AST is the liver and the kidney yin-deficiency type, accumulated blood stasis type, stagnation of liver-energy and spleen deficiency type, the spleen and the kidney yang-deficiency type, the retention of evil wetness-heat type. The sequence of HBV patients of ALT is the liver and the kidney yin-deficiency type, stagnation of liver-energy and spleen deficiency type, the retention of evil wetness-heat type, the spleen and the kidney yang-deficiency type, accumulated blood stasis type. The sequence of HBV patients of GLB is the spleen and the kidney yang-deficiency type, the liver and the kidney yin-deficiency type, accumulated blood stasis type, the retention of evil wetness-heat type, stagnation of liver-energy and spleen deficiency type. The sequence of HBV patients of TBIL is stagnation of liver-energy and spleen deficiency type, the retention of evil wetness-heat type, accumulated blood stasis type, the liver and the kidney yin-deficiency type, the spleen and the kidney yang-deficiency type. The sequence of HBV patients of TBA is stagnation of liver-energy and spleen deficiency type, accumulated blood stasis type, the retention of evil wetness-heat type, the liver and the kidney yin-deficiency type, the spleen and the kidney yang-deficiency type. There is no significant difference in groups compared(P＞0.05). In all cases we have studied, most liver function positive patients are belonging to the stagnation of liver-energy and spleen deficiency type. The sequence of positive syndromes in ALT,AST,GLB,TBIL,TBA is: stagnation of liver-energy and spleen deficiency type, the retention of evil wetness-heat type, accumulated blood stasis type, the liver and the kidney yin-deficiency type, the spleen and the kidney yang-deficiency type. The sequence of HBV patients of PCⅢis the liver and the kidney yin-deficiency type, the spleen and the kidney yang-deficiency type, stagnation of liver-energy and spleen deficiency type, accumulated blood stasis type, the retention of evil wetness-heat type. The sequence of HBV patients of LN is the liver and the kidney yin-deficiency type, stagnation of liver-energy and spleen deficiency type, accumulated blood stasis type, the retention of evil wetness-heat type, the spleen and the kidney yang-deficiency type. The sequence of HBV patients of CI-Ⅳis accumulated blood stasis type, the liver and the kidney yin-deficiency type, stagnation of liver-energy and spleen deficiency type, the retention of evil wetness-heat type, the spleen and the kidney yang-deficiency type. The sequence of HBV patients of HA is accumulated blood stasis type, stagnation of liver-energy and spleen deficiency type, the spleen and the kidney yang-deficiency type, the retention of evil wetness-heat type, the liver and the kidney yin-deficiency type. In correlation analysis, ALT and AST had correlation with HBV-DNA(10~6 copies/ml -10~7 copies/ml). Different levers of replication of HBV-DNA in PC-Ⅲ, LN, HA, CI-Ⅳ, is not obvious. There is low lever correlation between ALT and CI-Ⅳ(P＜0.05). It is the same with AST and CI-Ⅳ.Conclusions:We can draw a clear conclusion: the Stagnation of liver-energy and spleen deficiency type is the most common in our clinical study. The stagnation of liver-energy and spleen deficiency type, the retention of evil wetness-heat type, accumulated blood stasis type, the liver and the kidney yin-deficiency type, and the spleen and the kidney yang-deficiency type are respective are the different stages of chronic hepatitis B. When the disease become worse, the lever of examination increased, but the cases decreased.
