Objective To explore the association of SNP+276 in second intron of adiponectin gene with type 2 diabetes mellitus and Macroangiopathy in Chinese. The correlation between the T allele and HOMA-IR,TG, fasting plasma insulin was investigated as well.Methods The adiponectin +276G/T polymorphism was identified by polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP) in198 type 2 diabetes patients and 98 healthy control subjects in Chinese. Total cholesterol,serum triglyceride and fasting plasma glucose were measured by CX7 Beckman and Fasting blood insulin was measured by radioimmunoassay andHOMA-IR:In (FINS xFPG/22.5)were calculated. Surm adiponectin level was assessed by ELISA. Blood pressure (BP), height, body weight, waist and buttock were measured, and body mass index (BMI) were calculated. Alleles and genotypes were assessed by chi-square test, andcontinuous variables by analysis of variance and q test, correlations of multiple variables by logistic regression analysis.Results ① The frequencies of +276TT homozygotes significient increase in control subject than type 2diabetes.There was no difference in the distribution of genotype between type 2 diabetes with macroangiopathy and simple type 2 diabetes.②comparison in clinical data between diabetes group and control group :Age , sex and BMI was matched in the two groups. BP , Fib, FPG and FINS in T2DM groups were significantly higher than those in control group (P<0.05). otherwise, Serum adiponectin level was lower in T2DM than those in control. ③Compared with type2 diabetic patients without macroangiopathy group, the diabetic patients with macroangiopathy have much higher levels Year(64.77±8.41 vs55.72±11.66, P<0.01) . SBP(148.84±21.87 vs 138.24±20.43 ,P<0.01),history of diabetes (11.68±6.11vs7.91±5.98,P<0.01)and hypertension(8.39±10.06vs3.3±7.23,P < 0.05), lower HDL-C(1.15±0.26vs1.67±0.85, P < 0.01)and adiponectin level (3.5±0.8vs4.8±1.9 P<0.05) .④The clinical and biochemical data in different genotypes were Significient different inFPG(11.4±4.12vs 9.59 ±3.21vs8.44±3.53) Fins (6.4±2.11vs6.86 ± 3.23vs4.76± 2.78) CH (5.11 ±0.91vs 5.12 ±1.21vs4.18±2.84)(P<0.05). FPG (11.4 ±4.12),CH(5.11 ±0.91), FINS(6.4±2.11)of GG genetype were significantly higher than those of the TT genetype(8.44±3.53, 4.18±2.84, 4.26±2.78respectively); Fins(6.86±3.23), HOMA-RI(4.36±1.88) and CH(5.12 ±1.21) of GT were higher than those of the TT gene type(4.76±2.78,3.23±2.86, 4.18±2.84respectively)( P< 0.05).⑤ Logistic show that FPG(OR=2.599 ,P=0.025),HOMA(OR=8.944,P=0.012) and genetype(GG+GT) (OR=15.7,P=0.021) were the major risk factors of diabete. The major risk factors of macroangiopathy was year(OR=1. 114,p=0.001) HOMA-RI(OR=1.45,P=0.012) ,history of hypertension(OR=1.101,P=0.005)and history of diabetes milletus (OR=1.124,P=0.037) .Conclusion The result suggested that the +276 G>Tpolymorphism of the adiponectin gene were association with type diabetes in Shandong Han population , with the +276TT homozygotes have a lower risk of type 2 dibetes than carriers of other genotype. The the +276 G>T polymorphism has no association in type 2 diabete with macroangiopathy.
