Objective To compare the effects and the cost of operative laparoscopy with that of laparotomy on ovarian cyst.Methods Ninety-two cases of ovarian cyst in Qi lu hospital of Shan dong University from Jan to Oct in 2006 were reviewed. The cases were classified into two groups according to the operative approaches: laparoscopy and laparotomy. Eleven clinical measures and three cost measures were analyzed.Results The average duration of operation in laparoscopy group(60.60 ±26.87 min) was insignificantly less than that in laparotomy group(68.17 ± 28.91 min) (P>0.05).The mean blood loss of 32.54+34.31 ml in laparoscopy group was significantly less than 65.12 ± 64.94 ml in laparotomy group (p<0.05 ) .The time of extubating urinary canal were similar in two groups (no significant difference P> 0.05).The recovery time including breaking winds' time , the bowels'movement time , post operative intravenous antibiotics time and post operative hospital stay time in laparoscopy group were significantly shorter than those in laparotomy group (1.77 ± 0.48d vs 2.05 ± 0.39d, 2.61±0.83d vs 3.38+0.88d, 3.33 ± 1.16d vs 4.69±1.70d, 3.92±1.08d vs 5.93 ± 1.75d, p<0.05 ) .The operative cost , anaesthetic cost and hospitalization cost were significantly more in laparoscopy than in laparotomy (2635.20±489.20 yuan vs 961.00±29.07yuan, 1543.79±213.42 yuan vs 889. 96 ± 369.65yuan, 9272.14± 1115.52yuan vs 6821.79±1369.43yuan ,p<0.05 ) .Conclusion Laparoscopy is a better choice for ovarian cyst than laparotomy in intraoperative bleeding and recovery time, but the cost of hospitalization in laparoscopy is more than that in laparotomy. As viewed from health economics, laparoscopy is a better choice for high-income people, and further study could tell us the definition of the high income. Nowadays, the popularization of laparoscopy demands decreasing the cost of laparoscopic instrument and anaesthesia.
