Comparison Of The Clinical Efficacy Of Laparoscopy And Laparotomy For Early-stage Ovarian Cancer

Objectives:To compare outcomes of different surgical methods in the treatment of earlystage ovarian cancer,and to explore the value of laparoscopy in Comprehensive surgical staging for early-stage ovarian cancer.Methods : Selected 73 patients with early-stage ovarian cancer between January 2012 to November 2017 in the Department of Gynecology of The First Hospital of Lan Zhou University for Comprehensive surgical staging as the research subjects,according to the surgical methods divided into laparoscopy group(33 cases)and laparotomy group(40 cases).Then compared the difference between two groups of operation time,intraoperative blood loss,total lymph nodes retrieved,blood transfusion rate,uterus resection rate,intraoperative organ injury rate,tumor rupture rate,return of bowel movement,postoperative hospitalization time,differential value of hemoglobin and albumin before and after operation,cancer cell positive rate of ascites,postoperative complications,postoperative chemotherapy,recurrence rate and mortality rate.Results:Compared with the laparotomy group,the laparoscopy group had :(1)significantly less intraoperative blood loss(189.1±155.3ml versus 373.8±198.8ml,P<0.05),(2)more total lymph nodes retrieved(29.4±8.7 versus 24.5±7.4,P<0.05),(3)less blood transfusion rate(3.0%versus 22.5%,P<0.05),(4)shorter postoperative hospitalization time[7(5-22)days versus 9.5(6-30)days,P<0.05],(5)less differential value of hemoglobin before and after operation(16.6±12.4g/L versus 24.3±14.8 g/L,P<0.05).The follow-up time of laparoscopy group was 30.1±19.8 months,and the follow-up time of laparotomy group was 39.3±2.5 months.During the follow-up period:(6)there was 1 patient(3.0%)died in laparoscopy group,no patient died in laparotomy group,and the difference was not statistically significant(P>0.05);(7)The recurrence rate of laparoscopy group and laparotomy group was 3% and 7.5%,respectively,and the difference was not statistically significant(P>0.05).Conclusions:The treatment of Laparoscopy comprehensive surgical staging for early-stage ovarian cancer is safe and reliable,with less bleeding,more lymph nodes retrieved and shorter hospitalization time,with clinical promotion price.
