After the PRC was founded, the Communist Party of China and the Chinese government attached great importance to the reform and development of China's medical and health programs, and gradually built a three-tier public and rural health service network based on the public ownership system. The resulting healthcare security system, which took the forms of government reimbursement, labor security and cooperative healthcare, provided all members of society with free or low-priced medical services, thus achieving healthcare results hailed by the world. Since China adopted reform and opening-up policies, China's economy has been growing rapidly, resulting in segmentation and restructuring of China's original medical and healthcare system; the government also reduced or eliminated certain functions in medical and health administration, resulting in a vacuum and disorderly state. Consequently, medical and human resources are in irrational distribution, the social equality of medical services is low, medical services are increasingly unavailable and unaffordable, production and marketing of drugs and medical equipment are in chaos and products are overpriced, the functions of the original rural grassroots healthcare networks have been reduced, and hospitals are becoming increasingly commercialized and privatized. All these problems presented fresh challenges to government regulators.In view of all these problems, under the market economic system, what role should the government play in the medical and healthcare sector, which has a direct bearing on the life of each and every person? Is the government supposed to reduce or abolish its duties? How can the government fulfill its responsibilities and role? These are the questions to be answered by this paper.This paper consists of our parts:Part I introduces the basic theories of the medical and healthcare system and government responsibilities, including the meaning of government responsibilities and public goods, the connotation of the administration of the medical and healthcare system, and the responsibilities of the Chinese government in the medical and healthcare system.Part II elaborates on the guiding role and achievements of the Chinese government in administrating the medical and healthcare system, and gives a description of the reform and development of the medical and healthcare system, and severaladministration models of foreign governments, particularly those of Britain, Germany, Singapore and Brazil, with regard to their medical and healthcare systems.Part III discusses the lack of government responsibilities in healthcare administration and its consequences, including the responsibility for ensuring social equality, improving laws and regulations, and enhancing medical security and administrative supervision and management.Part IV explores the measures for improving the responsibility mechanism of the Chinese government in the medical and healthcare system under the market economic conditions. Such measures include reforming mindset, clarifying objectives, formulating health plans, improving health administrative legislation, establishing rules and regulations for medical and healthcare administration, enhancing the medical security system, ensuring universal access to basic medical services, strengthening government functions, boosting organizational building, and intensifying government administrative supervision.
